Question : Hi, I have a history of reproductive cancers in my family, and I have irregular spotting between periods. This last cycle I have bled for 22 days. I am extremely worried about this and have ability to private pay for either a CT or an MRI. Please advise to what the best testing would be. Or is a hysteroscopy better? In order of what test would be the most accurate to see cancers, could you please list the order of testing.

Hi,
Welcome to 'Ask A Doctor' service.
I am Dr. Ramadevi Wani. I will be answering your concerns today.
Please give me the details of cancers in your family and at what age they had. That gives some idea as to what risk you are exposed to.
Do you have postcoital bleeding? What was the Pap smear result? You should get a colposcopy done.
Hysteroscopy and D&C are needed for this bleeding. If you have not tested for thyroid and sugar test, get it done now. Also, CA 125 should be done.
Based on the results of the ultrasound, we may think of further evaluation.
If you have further concerns, I will be happy to answer.

The pap smear hasn't come back. Sister diagnosed cervical cancer at 35.
Grandmother at 40.
I'd like to know between the MRI and the CT scan what would be the best test for imaging. OR is the colopscopy better?
No post coital bleeding.

Hi,
Welcome back.
With the history of your sister having cervical cancer your chances of developing the disease are 2 to 3 times higher than if no one in the family had it.
Colposcopy biopsy is enough for the time being as you do not have symptoms. Be regular with your smear tests. Pap smear with HR- HPV virus testing is better than Pap test alone
CT or MRI is not needed.
If you have further concerns, do write to me.
