Question: Hi. I have had a dull headache since using mdma for one night on july 20th. This is concentrated at the back of the head. The pain has never been severe. I have also had some vision issues since then (trouble reading screens). I had some dizziness in the week following but that has mostly subsided. I still feel like I get lightheaded more easily when getting up from a sitting down position. I have had anxiety about the headache which may be contributing to the stiffness in the neck. I am worried about the headaches and would like to know if I should go to a hospital for this. Thank you so much for your help.

Therefore, I think it would be a good idea for you to consider going to your doctor and requesting a CT of the brain...(or better yet an MRI) just to rule ouot the possibility of a small bleed either in the brain itself or in the ventricles of the brain causing a slightly ELEVATED intracranial cerebrospinal (CSF) pressure.
I don't believe it is necessary at this late date to be going to any emergency rooms BUT getting an order for an imaging study for the brain is a very reasonable action to take under the circumstance. If your symptoms begin getting more intense or other symptoms start popping up that you didn't have before then, I would consider an ER visit since it would be very unclear as to what were going on and a more formal workup to identify some type of intracranial architectural or structural problem may need to be undertaken. Here is some additional information I can provide that may be helpful.
On the night of July 20th, I took a total of 1.5 tablets of MDMA. I have not taken any tablets since then, or any other illegal substance.
I took 1/2 tablet each "dose", approximately 2 hours apart. So one-half tablet around 8pm, one-half tablet around 10pm, and one-half tablet at 12am. Throughout the night, I had 2 alcoholic beverages (white claw hard seltzer 5% alcohol) from about 9pm - 1am
I have used MDMA only lightly (3 tablets) in the past year and and never had adverse symptoms persist for more than one day. Previous adverse symptoms consisted only of tiredness, lethargy, and insomnia on the night of use.
During each previous use, I only had ONE tablet total (again, broken in half and taken 2 hours apart). My previous dates of use were XXXXXXX 30, 2018. Mid August 2018. and October 31, 2018. During these uses, I had one or zero alcoholic drinks. I had not used any MDMA or any other illegal substance since Nov 2018, so it had been a full 8 (almost 9) months since my last MDMA use. I do not smoke marijuana or take any prescription medication. I typically have 2-3 alcoholic drinks per week, on weekends.
Since taking MDMA on July 20th, I have not had any alcohol and have not had caffeine. Before this, I would generally have an espresso and 1/2 scoop of pre-workout daily, so generally about 100-200mg of caffeine daily. I'm uncertain if caffeine withdrawal is a factor here. I have read that symptoms do not generally last longer than two weeks for caffeine withdrawal, though I do have a high sensitivity to the effects of caffeine (it makes me pretty "wired").
I should note that vision/focus has improved since after the first week, but I am still experiencing a small amount of eye strain, mostly in my left eye. I have had an eye exam and eye imaging completed. I did have a small retinal tear in the periphery of my right eye, though this was almost certainly caused by extreme nearsightedness and astigmatism (-8.50 myopia). I am currently in a dry climate (Los Angeles), so again, dry eye could be the culprit for strain and slight sensitivity to light. I will be returning to a more humid climate (Cleveland, Ohio) on August 23rd.
I also wanted to mention that I am feeling very ashamed of my decision to ever take this substance. I have a Bachelor's degree in molecular biology and should know better than to take a substance with the possibility of adverse side-effects. I take my health very seriously and this lapse in judgement is causing a lot of emotional distress. I sincerely appreciate your help and advice in treating these symptoms and moving forward. Many thanks for the additional information. As the old saying goes in my business, "If things don't seem as they should- either ask a few more questions or let the patient give you a few more answers!"
And then, you returned the replay with information that more easily explains some of your initial complaints that didn't seem quite right assuming MDMA was the only drug involved and that there had been no other history of either MDMA use or even other drug/alcohol ingestion. However, that is rarely the case in real life and in fact, you very generously did provide more complete background information on your prior use of MDMA as well as combining with alcohol in the past.
I ran an addiction outpatient facility actually in Bainbridge, Ohio for nearly 2 years and therefore, can recount stories to you about the young adults I managed and treated who used MDMA. As an educated and accomplished young scientist yourself I probably don't have to point out to you that this substance has not enjoyed the same degree of intensive research as other drugs such as cocaine or marijuana. Still, we understand some basic things about this substance and how it can interact with other drugs, alcohol, and caffeine.
I can tell you that there is NO HARD CORE scientific proof by way of standardized test results universally accepted by the majority of experts regarding a number of facets of MDMA use, however, I can tell you anecdotally as a treating physician and neurologist from the clinic that I've seen a vast array of responses of people who have used MDMA and I am convinced that there is a LARGE POTENTIAL for actual neuronal damage in the brain. Support of this contention comes from several larger scale clinical studies both with animals and human subjects and part comes from abnormal PET imaging studies that are shown to persist in some users (even after just several usages) for numbers of weeks and months.
Furthermore, these imaging changes of the brains that have been studied (some of them ones that I've treated and managed) are likely showing damage to SEROTONIN and NOREPINEPHRINE neurotransmitter systems involving both the massive release of these neurotransmitters as well as sabotaging the reuptake systems that metabolize these powerful chemical substances and therefore, causes an affect akin to BURNING NERVE CELLS out....
The symptoms you are experiencing (as I said in my previous response) are more likely than not due to the ABUSE of the MDMA (and unfortunately, as a doctor I must call a spade a spade, right?). What you may not be able to appreciate is that once the HIGH effects seemingly wear off and a person feels they are back to normal....the chemical changes that are promoted by the presence such unbalanced amounts of norepinephrine and serotonin in the system lead to a cascade of other neurotransmitter and even hormonal changes that can become permanent targets of change both in the brain and other organs that can make giving up the substance extraordinarily difficult.
The most well respected methodologies used to help people overcome MDMA use and return to more normal central nervous system activity include BIOFEEDBACK, COGNITIVE BEHAVIORAL THERAPY, and then, other methods have also shown some promise such as Tai-Chi in combination with CBT, acupuncture, and EDUCATION.
You mentioned that you are returning to XXXXXXX Ohio in the next couple of weeks. You should know that I have an office in Parma, Ohio right in front of the Parmatown Mall and across the street from used to be called PARMA COMMUNITY GENERAL HOSPITAL. I actually do exercise every day - I either bike 12-20 miles or run 5.5 miles (Runyon Canyon in West Hollywood). I've been exercising regularly in this manner since February 2018.
These 4 instances of MDMA use in the past 13 months are the only illegal drugs I've ever used in my life, so I feel strongly that I am capable of putting that behind me. I've always been afraid to try illegal drugs (hence my first use being at age 29), and it turns out that was a very smart thing to be afraid of and that rational fear has certainly returned.
The people I spend nearly all of my time with (my two roommates and best friends in Cleveland) do not use illegal drugs either, so any temptation and/or "ease of access"/peer pressure is very low. The person who introduced me to MDMA was actually my brother who lives in LA, and I do not spend a whole lot of time with him, as we just don't really talk or hangout much. No animosity, just not much in common...(I have 6 other siblings I'm closer to).

I don't doubt that you have the ability and motivation not to use in the future....in fact, how coincidental is this: Just saw a young lady in my office (30 minutes ago) who is a mere 22 years old but was a former HARD CORE drug user....everything including heroin and every pill known to man, smoked anything that could possibly be smoked if it messed with the mind, drank anything that could potentially knock her out or make her wired.....until 18 months ago, she decided that she had lived the wrong lifestyle and been around the wrong people for too long, had hurt her family, herself (been physically/sexually abused, been to jail, committed crimes to sustain her habits, lived under every bridge and shag hole by the River and around the lake) and decided.....18 months ago that was enough.....she's been sober since that time....just went cold turkey and hasn't looked back.....so...if, a 20 year old can have that much resolve and insight to how messed up her life had become, and much she had done in such a short time to hurt many other people (some innocent of any wrongdoing against her, etc).....then, it stands to reason that someone with your intellectual capacity and insight should be able to do as well....
Still, the fact that you DIDN'T really stop after the 3rd time, or the 2nd, or the 1st time....and the fact, that this time around you had combined things with alcohol usage and so forth....well, I would be remiss if I didn't sound the alarm bell and wave my hands around like Will Robinson's Robot from Lost in Space does yelling, 'DANGER! DANGER! DANGER!" and see if there were some way we could cement the deal so that you never feel the curiosity or need to use illicit substances again. Just take the DEA and the FDA's word that things such as MDMA, Kratom, bath salts, and any number of other designer types of drugs are really and truly of no medicinal value, and in fact, should be avoided for one's own safety.
I'm happy to have been of some measure of help and am confident that you do in fact, have the machinery within to put a stop to unhealthful usages of substances and the creativity to channel curiosities or restless urges in other more productive directions.....such as playing RUGBY with the Rovers?
