Brief Answer:
Visit orthopaedic doctor.

Detailed Answer:
Hello dear , welcome to Ask a doctor service.
I read your query and here is my advice.
Your doctor should order you blood uric acid levels and ERS before diagnosing you with gout. Most characteristic for gout is toe and not thumb.
I think it is a carpal tunnel syndrome.
It is the entrapment of median nerve.
You should have rest, use ice compresses, and support splint and use naproxen for pain. You can visit an orthopaedic doctor.

Hope I have answered the question.
