I recently got this blood tests back yesterday.
They show some thyroid issues. I have not been able to visit a doctor yet, plan to get an appointment by Tuesday.
What i want to know is if i am safe to exercise in the gym with these levels before starting medication for thyroid? As said i can't get in with my GP till tomorrow afternoon or Tuesday and i have a personal training appointment tomorrow morning.
Please give opinion. Blood tests have been uploaded.
Thanks
anything strenuous should be avoided, immediate treatment required...

Hello and Welcome to 'Ask A Doctor' service...
I've gone through your query and the reports in details...
I can understand your concerns...
Well, your reports, as you've already pointed out, is suggestive of extremely depressed/suppressed thyroid gland functioning...
If you're symptomatic (experiencing any dizziness, palpitation or rapid heartbeat, sluggishness, lethargy etc.), I'd not recommend any kind of gym training or any strenuous activities at all till the necessary treatment (Thyroxine supplementation) starts. The elevated Prolactin level can also be secondary to this dysfunction.
In fact, waiting for another two days can even worse this further.
Not to panic though, if you can't get it in touch with your GP, then I'd recommend visiting the nearest Urgent care simply for a prompt check-up (not only they'll provide you with the necessary medications but also they may run a few more investigation panel (namely an Ultrasound scan of the Thyroid gland, Urea and electrolytes, anti TPO antibodies) to look into it in details.
Let me know if I can assist you further.

Thank you for your response, I have to admit I am in a small stage of panic!
What really concerns me is that you say even waiting two days could make this worse. As its been 4 days since I had the blood taken, tomorrow will be day 5! Do you think I'm safe leaving this until tomorrow? I'm sure I could get some kind of GP appointment then but it wont be until the afternoon I think. But that's 5 days since bloods! I could go to the accident and emergency at my local hospital now but it would be great if I wont be putting myself at risk by waiting until tomorrow. Can you please advise.
I am experiencing symptoms mainly extreme tiredness (Just sat on the cough all day today on the laptop). Also experiencing light headedness and some occasional dizziness. Just feel a bit zoned out if that makes sense.
Oximeter shows my blood oxygen level as 97%, heart rate is 74-80bpm
Can you please tell me the main risks of exercising before receiving treatment for the underactive thyroid? E.g if I was to train in the morning what is the worst that could happen? I've heard people online say everything from "you will be fine exercising, it helps underactive thyroid" all the way to "you could end up in a Coma" !
Thanks Dr Banerjee :)

Welcome back,
Thanks for all these details...
See XXXXXXX it's an entirely individual thing in whose case it may go worse and whose not...Even in our clinical practice, we have seen patients in severe conditions (i.e. Myxedema coma) at much much lower range of TSH compared to yours...
Now, since it's 5days already, waiting for another day probably could not be any worse, but provided you stay at home, take rest, eat healthy, not to exert yourself too much which can only put your body at the higher level of stress during this period (which in turn can actually deteriorate these above mentioned symptoms further)...
Regarding the exercise, it's about putting your safety at first...If you feel ok otherwise, you can go for light stretching and freehand exercises but not anything which involves lifting heavy pieces of machinery and weights (the possible risks are endless, discussing them can only increase anxiety level further, which is certainly not recommended). So don't worry, but just follow this for the time being...
Once the treatment gets started, within a few days only you should be noticing the change in yourself, and eventually, begin your full-fledged gym sessions...
Best Wishes...

Thank you doctor, you have been an incredible help with everything.
Well I could not get a doctors appointment today but they squeezed me in with the advanced nurse practitioner. I feel very unsure about all this now, because honestly he did not sound like he had any clue about thyroid. He looked at my blood results and said he could not tell if I had an underactive or overactive thyroid! Then he said he would speak to the doctor. So he seen the doctor then came back to me and said that I have an overactive thyroid and they would be prescribing me thyroxine.
My reports clearly show underactive thyroid and they would never prescribe thyroxine for an overactive thyroid, right? So I honestly feel like he has no idea what he was talking about.
Nether the less they issued a prescription for thyroxine... But they said that I can not pick the prescription up for the thyroxine until tomorrow? Will I be okay until tomorrow do you think?
The advanced nurse practitioner really acted like this was no big deal. He said I had no risk of Myxedema coma and said that I would be fine. He also said that I could exercise as normal with no risks.
Whats concerned me also is that they don't even want to check my thyroid levels (T3, T4) again, he simply printed out two new blood tests that he said I should go for in 6 weeks. One of them is for prolactin and the other for TSH.
I'm honestly more scared now than before I went to the doctors! Can you tell me what you think about all this doctor? I mean I definitely have a prescription for thyroxine waiting for me tomorrow morning, but I'm still concerned about between now and then.
Do you get any warning signs of Myxedema coma? E.g does it come on slowly or is it a sudden thing? Would I notice vital signs such as heart rate and blood pressure changing leading up to it? Or is it sudden.
Thank you again
follow as already advised...

Welcome back,
Well, it's unfortunate that you couldn't get hold of your GP and end up to the Nurse practitioner. The flight is flown by the pilot and not by the flight-attendant, isn't it? So, just get hold of your doctor as soon as you can and get evaluated further...Meanwhile, it's of no use to discuss on what they think and what not, so kindly don't focus on that.
As already explained earlier, since it's been 5days already, waiting for another day probably could not be any worse... Just bear with this moment till tomorrow by staying vigilant. Please don't panic, it'll only add on to the anxiety level. Just try to switch the focus on to something else, watch some games, movies, listen to music, keep yourself busy.
No, no one can predict when and how it may come, moreover, let's not think about it like that as it can only make you over-anxious regarding each and every minute symptoms.
Just keep the mind busy with something else, hopefully, you'll get hold of your GP and everything will be ok soon...
Hope this answers your query...

I will try and keep my mind off things and relax for the night.
Just one final thing then I will rate and review your answers :)
I seen you said I should get in with my GP as soon as possible. But today the nurse practitioner gone into my GP and explained my case to her, my GP was the one who is issuing my prescription for thyroxine tomorrow morning (Although I haven't seen her).
Do you think I'm fine just following the GP and nurse practitioners advice and just taking the thyroxine tomorrow? E.g do I really need to see my GP or will simply taking the thyroxine fix my problem?
Thanks
I believe your GP's been in the loop only then...
Detailed Answer:
Thanks for mentioning this details...
Well, then I believe your GP's been in the loop only, since the prescription is being issued from her end only, and must've gone through your reports as well as...
Yes, let's just start taking the Thyroxine, and monitor your progress day by day...Too early to say if anything else will be required, if at all, then it'll be advised accordingly...let's get on with this first...
Meanwhile, you can meet her as per the availability and discuss face to face afterwards. If you feel they're not getting to it as much as they should, there's always another way available (getting a referral for an Endocrinologist and proceed)...
Don't worry XXXXXXX take rest now...
Kind Regards
