Question : Hi, I recently started having a bump on my buttocks and it is quite painful, I have attached a picture, let me know what you think it is and what I should do. I have a history of Type2 diabetes, Pilonidal cyst, and maybe prostatitis (experiencing frequent and painful urination)

Have seen your details and I appreciate your concerns.
1)I have seen the picture attached by you.You have lots of hairs around your anus and on butts which may result in condition called pilonidal cyst.The cyst is connected directly to rectal canal via many routes. If we see the anatomy of rectum and urinary tract(Prostate lies just on front of rectum) so a long term infection of rectum may result in direct infection of urinary tract/ Prostate causing Urinary tract infection/Prostatitis.

I would suggest you to visit a MS general surgeon in Person and get you ano rectal canal investigated by proctoscope as well as per rectal examination by finger.Further tests like MRI could be suggested to track the route of sinus.
A urine Routine microscopy and culture test along with Complete blood count Stool routine microscopy,ESR and CRP should be done to rule out degree of infection.
What medicines are you having right now?

I just finished zithromicyn 2 days ago and amoxicillin 3 weeks ago. Taking metformin insulin and Lipitor daily.

Should I be very concerned about this?

Definitely yes you are diabetic patient and to control any infection in diabetic is 10 times difficult then non diabetic person.
Infact today we are going through corona pandemic and diabetics are at high risk of catching infection.
Kindly mention the dose of Zithromycin and duration?
Same with Amoxicillin and duration?
How much benefitted do you feel after that?
Who suggested you to go for them?
Provide a detailed history event wise for a better suggestion.

Zithromicyn was 250mg (500mg the first day) and for 5 days which ended on Thursday, and Amoxicilin was I think 500MG 2x a day for 7-10 days in early April. I am more concerned that this came after I ended the doses (or maybe one of them caused it?), if it was an infection wouldn't these have fixed it?

These symptoms on my butt started a few days ago but I really noticed it 2 days ago. I checked this morning and it looks the same but feels a little more sore.

Oral antibiotics never works if there is pus inside the wound.
Pilonidal sinus needs to be drained usually.
I would suggest you to visit a MS general surgeon and follow up after that.
