She has had no nausea or vomiting or hypersensitivity to light/sound. She has had an issue for a long time with eye floaters and that continues and has gotten a bit worse over the last few months. She met with an optometrist a month ago but there was nothing they could find wrong.

As of now, we can treat her like migraine. Since the headache frequency is high, we need to start preventive medications to reduce the frequency and severity of headaches.

Propranolol is a good drug for that purpose. Usual dose is 40 mg once daily after breakfast.

For relief from headache, naproxen tablets can be used.

They have tried multiple migraine medicines at the ER with no success. We don't believe it to be a migraine. What else could it be?

Other possibilities to exclude would be vasculitis/venous thrombosis, etc

In about 30% of cases, other causes are found out on further investigations. Initial test is MRI with MR Venogram brain. This can show conditions such as vasculitis (inflammation of blood vessels) or cerebral venous thrombosis (clotting of blood in veins).

In some cases, it could be idiopathic intracranial hypertension, which can cause headache with visual disturbances. This can be found out by eye examination to look for papilloedema.
