Question: Hi, thanks in advance for your medical advice. My husband is 56 and just broke out in a sweat, soaking most of his cotton tee-shirt. He also became very tremulous, with both hands visibly shaking. He also felt lightheaded and, when I checked his bp, it was 156/93 after having taken his blood pressure medication today. For several months, he has been treated for anxiety and depression with Effexor, Buspar and Zyprexa, after experiencing numerous high stressors. Another off thing has happened about five times during the last 2 weeks - he has urinated on himself after feeling that he has voided urine completely. And, he doesn’t feel it happening until he looks down or I point out that his pants are wet. As his wife, I’m getting scared that something medical is being overlooked. He had 2 major heart attacks at 48 years old as an avid bicyclist. The last MI resulted in quadruple bypass surgery. I recognize I have some trauma from watching him during the heart attacks so I don’t want to overreact. And, my husband is a physician (now teaches medical students) who sometimes downplays his symptoms. I’ve noticed his tremulous hands, along with a pale face, many times and have dismissed it as anxiety. But, I just have a gut feeling that some medical issue may be being overlooked. What would you recommend as the next step? I know you can’t diagnose without an exam but can you share some possible medical conditions that could account for these issues? Any advice is much appreciated!

Dear Ma’am, I have gone through your question and I understand your concerns but these symptoms could be due to a number of causes.. The most common of which are prostatic disorders ( prostatitis or prostatic hyperplasia) ,urinary tract infection, diabetes, vitamin b 12 difficiency and hyperthyroidism.. Both diabetes and vitamin b12 difficiency can lead to incontinence by causing neuropathy of the nervous supply controlling urination..
So, I would recommend that he gets checked up to exclude these causes. Prostatic disorders can be excluded by a pelvic ultrasound, urinary tract infection can be excluded by a urinalysis, diabetes can be excluded by a blood test for blood glucose levels both fasting and postprandial together with a blood test for HbA1c levels, vitamin b12 difficiency can be excluded by a blood test for vitamin b12 levels and hyperthyroidism can be excluded by a blood test for thyroid hormonal profile including T3, T4 and TSH..
Another uncommon causes include cystitis and neurological causes. I hope you find this helpful.
