                                    Brief Answer:
Bowenoid papulosis
Detailed Answer:
Hello and welcome to ask a doctor service..I have gone through your query and viewed the image.
I would keep a possibility of bowenoid papulosis for these perianal flat, brown papules. It is caused by HPV and the infection is acquired through sexual intercourse.
Hope I have answered your query. Let me know if you need any more assistance.

How does this just arise? Are you sure this is what it is? Comparing to pictures I should have raised bumps? This is just all flat no lumps

Regarding bowenoid papulosis
Detailed Answer:
Hi.
Going by the morphology and recent onset bowenoid papulosis is a likely possibility. They can be minimally raised initially. I advice you to get it checked by a dermatologist or venereal specialist.
Take care.

In your opinion, is this the only thing it could be? Like I said I sweat a lot and honestly I do suffer from anal leakage..so I%E2%80%99m often left sweaty and sore..and this is everyday I sweat like this..it just doesn%E2%80%99t add up to me..for all I know the marks have been there for a while as I do not look at my anus often. Also because the last person that I had sex with was a long time ago and they were clean..a virgin when I met them

Regarding perianal lesions
Detailed Answer:
Hi.
This is the most likely diagnosis. The other possibility is of course genital warts or other STDs like syphilis (condyloma lata). Genital infection with HPV is often clinically silent so the fact that the last person you had sex with were clean on the surface doesn't rule out that they were not infected with HPV. Moreover HPV may cause visible lesions long after the actual infection.
Take care.
