Hi, Yesterday Had Sex With Women With Unknown Status. I
Question: Hi, yesterday had sex with women with unknown status. I was having condom and even after checked condom with water.It was intact but some white stuff greasy stuff was outside of condom and near the area of vagina. I m sure that condom was intact as when i put water, it was there with my semen. Please suggest me am i being of risk,. I have started tenofovir 300 mg and Lamividine 150 withing 22 hours of this incident as post exposure porphylaxis. But reading sideeffects not sure what to do. Need ur advice plz Sir...

As your condom was intact till the end of the sexual act and it held your semen, you need not worry at all. We are advising the discordant couples (one partner is HIV positive and the other negative) to have sex with condoms. All remain sero discordant for years after regular contact. Condom offers 99% protection against HIV infection. The outer sticky white material is from your partner's secretions. Not to worry. There is no role for two drugs combination in Post exposure prophylaxis for HIV infection as per present day guidelines. One has to take 3 drug Combination for PEP and to continue for 4 weeks. The basic HIV test is a must and has to be repeated every month for 3 months and one after 6 months. Only pre exposure prophylaxis has two drug combination. I feel you do not need PEP. As you have started, complete the course and the recommendations. Nothing to worry.

Thank you Sir, i have started tenofovir 300 mg and lamivudine 150 mg twice. Please suggest me the name of third one available in XXXXXXX to add to it. This is my first day with PEP.
I did mistake perhaps the blunder to engage in it in these days.
10 days ago, burnt my finger skin wile making tea. Photo attached, if it touches used condom, do u think it will change risk.
Further is there anything pin hole in condom, if its breaks generally open it in big way, correct?

You can add Tab. Instgra 50mg daily along with Tab Tenofovir and Lamivudine. Or you can take Tab. Viropil (combination of 3 drugs, 3 in one), marketed by Emcure, 1 tab daily for 28 days. As you have started PEP, not to worry about the burnt finger (10 days ago) or about the extra risk which is also negligible.

Kindly share with me and other suportive medicine with PEB that will help me to maintain or reducing the impact of pep on body. I m taking vitamin d n calcium, any other toadvice to minimize impact on kideys?

The combination of Tenofovir+Lamivudine or Emricitabine+ Dolutegravir has least side effects when compared to other ART drugs. That too, you are going to take only for 4 weeks. People are on ART (especially in this combination) for years together without any problem. So you need not bother about the side effects. Tenofovir will have problem only for individuals who have already impaired kidney functions and not for normal people. Dolutegravir may have mild dizziness in some individuals. You forget all these things and complete the course pf PEP. No other supportive drugs are necessary, If you have any other problems like gastric disturbances, report to your physician who suggested PEP for you.
