How Are Antibiotics Given In An Infant?

Question: Hi
My baby is 17 months old. She started to take antibiotics (Primacine Erythromecin) yesterday. She was fine with the temperature yesterday and at night. But this morning after I gave her antibiotics on empty stomach before food temperature raised up to 38 and her XXXXXXX was in pain. She was screaming and breathing quickly. She is shaking as well. I have given her Ibuprofen. Looks like it is settleding down.
1. Do I suppose to give antibiotics on empty stomach?
2. Why she has a temperature if she is on antibiotics?
3. Her eye is leaking and there is yellow discharge. Will these antibiotics help?
4. Her nose is blocked for a very long time. Will antibiotics help?
5. Do I give her antibiotics for 5 or 7 days? She has been ill for 10 weeks. Something does not let her to recover.

I am sorry to hear that your child is sick. The first question that I would like to ask is whether the antibiotic has been prescribed by a pediatrician or is it self medication. You should not self medicate in case of a child of such a tender age and you need to visit a doctor if you have not done it already.

Now, let me try to address the issues one by one.
1. The antibiotics or most other medicines should be given in full stomach and not while empty. Many medicines have a propensity to cause gastritis and hence this measure. Excess gastric acid secretion can worsen the situation and cause abdominal pain which could be the case in case of your child.
2. Most fevers in case of children are of viral origin. Often there can be a secondary bacterial infection and hence many doctors prescribe antibiotics to be on the safer side. But irrespective of the use of anitibiotics, a viral infection would take 5-7 days to resolve and it would resolve on its own. Hence, inspite of the antibiotics being given, fever is not unlikely. Additionally, other probable causes of fever like malaria and typhoid might also need to be investigated if felt necessary by your doctor.
3. The yellowish discharge from the eyes indicates an eye infection (which is probably independant from the cause of fever) and needs to be treated with local antibiotic drops like Ciplox (ciprofloxacin) 1 drop in the affected eye(s) 4-5 times a day. Oral antibiotics are less likely to be effective.
4. The nose block could be due to thickening of the mucus, often due to a viral infection. Applying saline nasal drops like nasoclear genereally relieves the block. If it is not helpful, you may try nasivion drops (oxymetazoline) a few times a day. Antibiotics are not required for this. But you need to continue the course of antibiotics since you have begun it.
5. If she has been ill for 10 long weeks, the cause of the fever needs to be investigated properly. Ideally giving antibiotics for 5 days is enough but it may depend on the severity of the infection as judged by your doctor.

The bottom line is that you need to be in the regular follow up of a pediatrician.

Thanks for reply. Antibiotics were prescribed by doctor.
Will not be giving them on empty stomach anymore.
She has been ill for 10 weeks. Every week one day she was fine, and after all started again. Doctors dont do anything. Just saying it is viral infection.

It seems inormal for UK doctors to have babies ill for many months.

Hi,

Thanks for writing back. There is some difference between what you just said and what you had said previously. Recurrent infections are different from continuous ones. In case of frequent recurrence, I too would rather suspect a viral cause as your doctors have done. Some children have a propensity to catch cold and I am sorry that your child is one of them. I would strongly suggest that you get a home nebulizer and whenever you feel that the child could have caught cold, nebulize the child with 2cc of normal saline or if wheezing is present, do it with a respule of duolin. Also ask your doctor to check if the tonsils are enlarged. Tonsils are lymphoid organs responsible for immunity against viruses and in case of frequent and recurrent infections tonsillectomy surgery might be required.
