You have 2 queries.I will answer the second one first.If there is evidence of gall stones on ultrasound,then the treatment depends on the number and size of the stones.If there are multiple or larger stones,then the treatment is surgery as the stones may cause blockage and jaundice. Surgery can be done open or laproscopically.

As for the first query,Hep E is a viral infection affecting the liver that is caused due to contaminated food. If there is jaundice then bilirubin levels are more than 2-3mg/dl. Your father's SGPT levels are very high and that is often seen in various liver disease. That occurs due to liver inflammation. At present there is no drug to cure hepatitis E. Supportive treatment is advised. Hospitalization depends on the patients condition and if unresponsive. The infection is usually self limited.

This case is complicated due to diabetes.Liver enzyme levels can keep rising to thousands due to severe liver damage.The cause for that needs to be found out.Although hepatitis is a major cause of elevated liver enzymes,your father possibly has multiple factors working against him like the diabetes.Alcohol abuse,if that was the case,would have contributed.Right now,the diabetes needs to be controlled.If the underlying cause of the liver disease is controlled,then the liver enzymes should come under control.

As for diet,he had to have a diabetic diet,with low on carbs,sugar,fat.If he is taking food by mouth,then the hospital dietitian should be able to help create a diet chart,depending on the sugar levels.Whole grains are better,vegetables are good.Apples,papaya,lemon,rhubarb,berries,melon.You should consult his doctor because he may be restricted to some foods on his advice.
