How Can I Reverse Dehydration Of One Of My Kidneys?

Question: Hi Doctor,
In my recent blood test doctor said one of my kidney is dehydrated. What is that mean and is it serious? How can I reverse that please?

Hi,

To be honest, there is nothing like one kidney is dehydrated, Maybe your doctor could have meant something else. What we understand is that, once our body is dehydrated, ie if the fluid is low in our body, our kidneys function can be compromised. For example we do urea, and creatinine as the tests for our renal function mostly, if these are raised we say that the kidneys are not working properly, and maybe a sign of renal failure, However, if our body is dehydrated (low body water content), in such cases the urea can be raised, but the creatinine is normal, or the urea is disproportionately high to the creatinine, here we say that the raised urea is not because of the renal failure, rather this indicates that our body is dehydrated.

So this is what your doctor might have said, that the blood tests may indicate the above, which means our kidney tests are bad not because of the kidney, but because of our dehydration. This is temporary, and the tests return to normal in little time.
The treatment of this is to replace fluid. This can be done either by taking a lot of fluid orally to keep ourselves hydrated or if the problem is severe we can also be given drips to replace the water.

This I suppose he meant if only blood tests are done, however, if he had done any CT scan or ultrasound, he might mean something else. But mostly it is this (explained above) when we tell someone that the kidney is dehydrated. If you still have any confusion you can share the tests here so to explain it in more clearer words.

My doctor did the following tests

Neck ultrasound scan

Abdominal ultrasound scan

chest xray

lung nuclear medicine scan

ECG

blood test as follows;-

U&E

Liver profile

TSH

CRP

Full blood count

ESR

Ferritin

Hba1c

He said the following,

All seems fine but I'm anemic so take iron tablets. And also my one side kidney is dehydrated and my d dimer test is higher than normal but not to worry. My grandma has a kidney cancer hence I'm little worried. Why my kidney dehydrated and why my d dimer test is high please?

If these tests were fine, especially if the urea and creatinine are normal, we are confident to say nothing major is wrong with your kidneys, and by dehydration of one kidney, he probably means you need to take more fluid. ( but I am still not sure what he meant, but your kidneys are ok if the tests are ok, still to be sure you can ask your doctor, or if you could share your tests with us if you have them )
Your concern about your grand-mom renal cancer is right, but if the ultrasound was ok, this is pretty reassuring, this means you should not worry about it.
Slightly raised d dimers may be there, and if you do not have any leg swelling or shortness of breath, this should not be worried about, I would just like you can repeat it after sometimes like in two weeks to see it should decrease.
I would personally advise that these tests are pretty reassuring and you should not worry, however you should repeat them in some time like a month, or more, to be sure that these minor problems like dehydrated kidney, slightly raised d dimers, and the low HB, should be resolving.

Thank you doctor, just one last question before I leave you please,

What causes the raised d dimers? because My legs(ankle area) does swell if I walk fast or for a long time then it'll go down if I rest. But the doctor said I don't have blood clot becuase the lung scan was normal. He also mentioned my d dimer is lower than my previous blood test which was done in Nov'17. But compared to normal level it's still high. What is the reason for this to be high and how can I reduce it please? I have ashtma, excema and raynauds condition.

Also to reverse my dehydration drinking 2L water per day is enough or do I need to drink anythings specific please?

Hi,

In fact, there are many reasons other then clots that can cause a raised level of D Dimer, however, that only modestly raises the level.
Causes include, just to mention here a few, like trauma, surgery, liver disease, heart diseases, pregnancy, or any inflammatory condition in the body,
I think this may be related to your diseases, like asthma, and eczema and Raynaud may all slightly increase the D Dimer level.
Sometimes, raised triglycerides, or raised uric acid, or RA factor may also show falsely high D dimer. However, in all these conditions, the value is not very high, while in blood clots, the value is very high most of the times.

Regarding fluid, this depends on person to person, and the climate as well, but for most of us, 2 to 3 liters of fluids per day (not only water, you can take other fluids like juices etc) is enough.
