Question: my friend has had a severe bloody nose I mean severe blood everywhere and won't stop for two days and headache
he is out of town what could be problem

Well, at his age (56, I believe it is), this type of persistent bleeding from the nose along with a headache, needs immediate medical attention. The primary suspicion is uncontrolled hypertension or sudden elevation of blood pressure. So it needs to be measured sequentially if found higher than usual, then immediate treatment is required to bring it down.

This is the most important factor, and it needs to be assessed properly (blood pressure monitoring, EKG, a complete Hemogram without any further delay). If he's reluctant to go to the Doctor, please call up urgent care or take him to the nearest ER for a prompt check-up.

Secondly, there could be some unnoticed trauma to the nose due to either nose-picking or some other reasons. Other few causes are some structural nose anomaly, blood thinners/blood coagulation anomaly.

First of all, tell him to drink a lot of fluids, don't blow the nose, keep pinching the nose gently by sitting upright.
