i have this weird lesion on the head of my penis.
it is slightly painful when comes in contact with my pants.

Hello,
The lesion appears to be some localized fungal infection. But evaluation is needed to make diagnosis. If there is any recent history of unprotected sexual intercourse with any high risk female then that could be a blister but usually blisters are very painful and are large in size.
There is no discharge so we can rule out any serious STD.
Avoid using any hard soaps over penis and clean with water only. Avoid using tight underwear for some days. This should heal by own.

it is quite well demarcated and it is slightly painful when I press on it. Otherwise there is no discharge. It is not something like syphillis right? I've not had any unprotected sexual activity.

You have no history of unprotected sexual intercourse. There is no discharge so the risk of syphilis is rare. Though syphilis rash may mimic normal inflammatory rash and the rash is painful too. But in a lot of patients its associated with discharge from penis.
