Question : I have severe dry scalp that is very itchy and my hair is falling out in clumps. I have tried several medicated shampoos and nothing is working

You seem to have seborrheic dermatitis.

Scalp seborrheic dermatitis presents as redness, itching and yellowish, greasy scaling/ flaking of the scalp. Seborrheic dermatitis is a steroid-responsive dermatosis. I suggest you use a coal tar and salicylic acid based shampoo, every alternate day. In addition to this, I suggest you apply a topical steroid lotion e.g. clobetasol propionate lotion or mometasone furoate lotion. These are prescription medications and I advise you to kindly talk to your local practitioner for a prescription of the same.
