How Can Oily Skin, Open Pores And A Visible Glabellar Vertical Line Be Managed For Better Aesthetic Results?

Question: Consultation regarding Open Pores & Skin Lightning :

I have very oily skin(oil forms 20-30 mins after face wash) and multiple open pores on the cheek area of my face and also i have deep scar in between and just above eyebrows(Glabellar Region).

Oily Skin: (Attached screenshots)
1. Any medications to be followed in order to get rid of excess oil formation and have the skin hydrated without much oil seen on the surface.
2. Time required to see its effects.

Open Pores:
1. Medications to get rid of this open pores permanently and have a clean and clear skin.
2. Time required to see its effects.

Glabellar vertical Line (Shared Screenshot):
1. Medications(without fillers or botox) to levelize this area.
2. Time required to see its effects.

Please provide answers inline to the queries asked and also it would be good if the medications shared will be of
1. Less Side Effects
2. Immediate and Long term Effect
3. No Reversal Effect (If we stop medicines in between on seeing good results and after reaching our satisfactory levels, it should not strike hard back to same initial condition).

Regarding oily skin, pore size and frown lines

Hello,

I have noted your concern and I have viewed the images images.
Pore size is genetically determined and depends on the size of oil glands/sebaceous glands. The bigger the oil gland, greater the pore size and oiler the skin.Though it is not possible to reduce the pore size or skin oiliness permanently, however, for oily skin I usually suggest oral isotretinoin to my patients in a dose of 5mg once daily for 4-6weeks. It works to shrinks sebaceous gland size by more than 90% in 4 weeks and therefore reduces oil secretion. It's effect lasts from 1-3 months.
Another option is to take regular salicylic acid peels at an internal of 2-4 weeks. Salicylic acids dries up excess oil and also refines the pore size.

As far the glabellar line/frown line is concerned, it is a static wrinkle. Only Fillers would help. Not even botox. Fat fillers and PolyMethylMethAcrylate (PMMA) based fillers are longer lasting ones.

Oil Control:

1. Could you please tell me the exact tablet to be used with isotretinoin 5mg.

2. As you specified effect will last for 1-3 months on usage of 4 weeks. What will be the procedure after 3 months?

3. Does it have any reversal effect, will it worsen the initial condition?

Static Wrinkle:

1. How long does the specified fillers effect last?

2. cost/sitting and number of sitting involved in this procedure.

Open Pores:

1. if not permanent cure, Is there any treatment which would shrink and levelize the skin and how long will it take to see results

2. How long will this result last.

Skin Lightning:

1. This is what i missed in my question, could you please give some medications for this as well.

Brief Answer:
Regarding open pores and skin lightening

Detailed Answer:
Hi.

Isotretinoin 5mg (Sotret cap 5mg) once a day for 1 month. It is a prescription product. It would shrink sebaceous glands and reduce oil secretion by 90% on about 4 weeks. After 1-3 months of stopping treatment your skin will gradually return to its original. There is no rebound worsening with this treatment. For maintenance you may use a Glycolic acid based product e.g Cutiyt G lotion, once a day at night.
Fillers would cost anything between 10k to 15k depending on the amount that would be required during injection. It is a single sitting procedure and effect of a PMMA fillers lasts more than 3-5 years and even longer. Fat injections are also long lasting.
For skin lightening I suggest you to use a broad spectrum sunscreen during the day. At night you may use a Glycolic acid based cream e.g Cutiyt G lotion. This lotion would also serve to tighten pores.
These are all prescription medications and I advice you to kindly talk to your local practitioner for a prescription of the same.

I have few more queries.

1. "After 1-3 months of stopping treatment your skin will gradually return to its original" - Original in the sense will it be again oily like skin condition before starting medication?

2. For skin lightning, can i use Hydroquinone or Kojic Acid based cream? if yes, which one is suitable for my skin condition and when to use this cream. or any other skin lightening cream do you want me to follow.

3. Could you please suggest a better wide spectrum sunscreen as you suggested.

4. Which facewash is suitable considering my current skin condition. 1 of the dermatologist suggested me to use Spongysal FW and another one suggested Ahaglow FW.

5. Adalene Nanogel is presribed for improving skin texture, would you suggest me to use this at this point of time.

Currently i have multiple parallel suggestions, could you please simplify those and give me exact medications to be followed along with its schedule.

It would be better if you can reply inline to the questions asked.

Hi.

Answer 1. Yes.
Answer 2. I prefer a Glycolic acid based product in my patients because it is not only skin lightening but also exfoliating as well as refines pore size.
Answer 3. The face washes suggested to you are Salicylic acid and Glycolic acid based. They exfoliate as we as deep cleanse. They are good for you as you have only skin.
Answe 4. Adalene gel contains Adapalene which is mainly for acne. It has no effect on pore size or oiliness or skin texture. I prefer to use a Glycolic acid based product for improving texture.
