Hi,

Pain in penis only during erection indicates one of the following causes. Either the blood filling spaces are filled more than capacity. Exact reason may not be known. But known to occur with hyper-excitation, use of some medicines like sildenafil or so; starting of fibrosis in the internal tissue of penis.

This can be diagnosed by the following:
Actual palpation and examination by a general surgeon / urologist. If required high resolution Ultrasound and color doppler or MRI studies.

It is better to get the exact diagnosis to have an appropriate management.

Anything you feel on palpation by your own? Knots / painful area? Any medicines that you take?
