If I were your treating doctor I would recommend you to use Ketoconazole shampoo twice weekly and apply a lotion containing a combination of Clotrimazole and Cortisone on the itchy and scaly areas of the scalp once daily at night for 10-14 days.
Once dandruff reduces you can discontinue the lotion but continue with Ketoconazole shampoo.
