How Does One Diagnose And Medically Treat Unexplained Infertility?
Question: hi good evening doctor
i am married five years ago, me and my wife trying to have parenthood but its only like a dream, we have tried many doctors from the past five years, my age is 30 and my wifes is 27, and tried many medicines from gynecologist but not yet possible to have a child till now, last month my friend referred to see an infertility specialist in XXXXXXX and i consulted a infertility specialist there, dr referred all the previuos history of my wife and took some test on that day and another some test on day 2 of period. he said that everything seems normal but my wife has pcos in the right ovary. he said we can move for iui and done follicular study from day 11 to 14 before that he gave her tab.letrazole 2.5 fromday 3 to day 7 twice a day, and metital (metformin) for a month.. he seen my semen analysis report too and said everything seems normal for both... we have successfully done our iui on day 13 after that he gave duphaston 10mg tablet for 20 days twice a day...

MY QUESTIONS ARE?
1. WHY WE DONT HAVE A CHILD WHILE EVERYTHING SEEMS NORMAL?
2.WILL THIS IUI WILL GIVE US SUCCESS? FINGERS CROSSED!!
3.WHILE TAKING SCAN SHE OVULATED ONE FOLLICLES WITH LEFT SIDE. WHERE THE PCOS IS PRESENT? AT THE RIGHT OVARY ITS NORMAL WITH MSF
4.WETHER SHE CAN GET CONCEIVE EVEN WITH PCOS THIS MONTH?

Greetings
I have gone through all your reports.I can understand your situation.As you rightly said scan is normal.both tubes patent.ovulatiion happened in this cycle with medications.
IUI has only 10 to 15 % success rate per cycle.You have to do continuously 4 IUI s to say that it is not successful.
She may not be ovulating without medication and that could be the reason.
There are around 10 to 15 % of infertility couple has no reason,called as UNEXPLAINED infertility.
Now I suggest you to continue IUI for 3 or 4 times continuously or alternating cycles.
Your semen test has to be repeated.
You can consult an XXXXXXX or urologist to rule out any varicocele or any genital infection as the progressive motility is less than 36%.
