Besides that tricyclic (and other) antidepressants and anticonvulsants have shown a good efficacy and are the recommended treatment for this condition. Milder opioids like tramadol can be used for pain that can't be managed otherwise.
Please keep in mind that opioids may induce a strange condition that causes increased sensitivity to pain (despite their potent analgesic effects) and actually cause more pain. Alternative treatments for the pain also exist and perhaps should be tried to avoid dependance on analgesics.

You can always visit another doctor for a second opinion or you can contact a local lab and ask them to do the cultures. Not having examined you I cannot confirm or rule out a fungal infection. I can assure you though, that internal organ infection is not very likely because it usually has blatant symptoms, like fever for example. The nail, tongue and vaginal infection is not supposed to cause anything more than local symptoms.

As far as the cultures are concerned, I live in a very rural area, and the closest doctor is 30 minutes away. Apparently they haven't the ability to take cultures..don't know if it is from not having the proper equipment or whether they are just ignorant. But I am starting to wonder if there is a reason why my doctor doesn't want to send me to an infectious disease specialist, especially because I was diagnosed with Lyme disease and Rocky Mt. Spotted Fever in the last two years.
Would you have any insight on WHY my physician continually refuses to refer me to a specialist whom can perform these cultures properly?
Oh, I almost always have a low grade temp. of 100.1 or so...my blood pressure drops VERY low every few weeks. I wake up in sweats, and of course I am in immense pain even on potent pain meds.
And not sure where you are from, but in the US, you cannot just go to a lab to be cultured and you cannot see a specialist without a primary doc's approval.
I understand what you are saying about the fungus being localized, BUT I am sure you know that the yeast can get into the muscles and nerves and brain as well, which is where I am having all of my problems.

I've read your thoughts and I understand your concerns. Despite that I cannot help more than just say my opinion on the facts you presented. I can't comment on your doctor's decisions because I don't have all the data the your doctor has (including clinical examination data).
Your fever needs investigation and there are a lot of tests that have to be done, particularly if it is long-standing. Fibromyalgia has criteria for the diagnosis but - you're right about that in a way - it's also a diagnosis of exclusion. The doctor has to exclude other potential causes of the symptoms first.
I didn't know that you had to have a primary care doctor referral for the lab tests. Perhaps you're talking about insurance-covered tests. But I'm sure that you can pay for the tests yourself, without your doctor's approval. If this is not the case, then you should seek the opinion of another primary care doctor.
