Question : I had lost 17 pounds in 3 months and have suffered from anxiety for years. Hypothyroid, obese at 218, blood sugar runs less than 140 throughout the day if I check it. I am not a diagnosed diabetic but was told I was prediabetic so I started testing 4 times a day. Today my highest was 134 after an hour of eating a turkey and cheese wrap. My anxiety always starts from a burning in my gut and goes from there. Fast heart rate, short of breath, sweat -mainly feet and top lip and I make it worse because I think about it. I take Zoloft 50 bid and Xanax .5 tid prn. I also take 25 of metoprolol bid and synthroid .125 daily. I have hctz for as needed. I went to er 3 times 2 weeks ago, once to a small and they said anxiety, second day same, couldn't calm down, chest pain, short of breath, etc. So my husband took me to our heart hospital in Little Rock and they did cxr, ekg, cardiac enzymes, cbc and all were fine except anemic. Well 2 days later it was worse and I was having abdominal pain. I have the lap band and thought I might have another incarcerated hernia so I went to our big Baptist Health where my bariatric surgeon was. They again did ekg, lab, and ct of abdomen with and without contrast. Said I was anemic like 9.1 and 32 and they saw no signs of cancer and treated me for gastritis. Well 4 days later no better so I called a local urgent care and talked to APN and she said come let me test you for h pylori. So I did, Positive. So I just finished yesterday amoxicillin, clarithromycin and Prilosec 14 day course bid. I have to continue the Prilosec for 6 months. I am still having some nausea, which she gave me Zofran for and I took a 1/2 of 4mg tab a little while ago and the nausea has subsided. My question is how long does it take to start feeling better? What else should I do. I have done Mylanta and even pepto in the past days to help and it did at times. I have started taking iron with vit c.

Brief Answer:
Stop taking Omeprazole and start Dexilant. Continue all other medications.

Detailed Answer:
Dr. Hanif warmly Welcomes you to my virtual clinic HCM!
I have carefully worked through your case, and can realize your health worries. Being your physician, I will take care all of your medical concerns.

Coming direct to your main question; I want to assure you that the situation you have will soon be over. Different patients take different times depending upon the disease severity and all the associated aspects and commodities.

You are not diabetic and I was to give you a good news that diabetics take longer in such cases due to gastropresis (a complication of diabetes affecting stomach). You will get better soon. Your stomach is doing good.

Regarding h.pylori you are treated as recommended. You need no more for that. Just after a sometime you have to go back to get tested again.

Dexilant is a potent drug and it will work very fine for you. Instead of taking omeprazole start this more potent drug.

Your anemia looks to be due to blood loss, that may be due to stomach or duodenum ulcers that usually happens to be there due to h.pylori and associated hyper-acidity. It is good news you are on Iron and Vit C. It hopefully will cover the deficiencies. Also add up green leafy vegetables and broccoli, spinach, kiwi, dry fruits and Iron-fortified foods available in market according to your taste.

Hypothyroidism also contributes to the increased weight. The sweating you have also is due to hypothyroidism and anemia.

Anemia also accounts for the shortness of breath and weakness.Your hemoglobin is low and you have to improve your diet plan.

Have a wonderful time ahead. Hope I answered all of your questions. Let me know if you have any further clinical concerns or doubts. I will be more than happy to get back to you in no time. If satisfied here, please give a generous five star rating and don't forget to close the discussion.
For any disease you have, a timely follow-up after sometime regarding the treatment efficacy and to see any need to change the treatment options is very necessary. Keep a follow up some days after this visit session ends.
