Brief Answer:
Biopsy report is needed to comment.

Detailed Answer:
Hello XXXX and welcome to HCM.
Assuming that Urologist has discussed the possibility of biopsy report of prostate cancer with Mr XXXXXXX XXXXXXX The biopsy report has not been sent in this query.
If biopsy report is Prostate cancer, question of prostate surgery in his case, does not arise, unless he develops block in passing urine.
The reason being, cancer has spread outside the prostate gland, into the bones in multiple sites, as seen on the whole body scan.
Usually, a hormone therapy medicine may be given, for delaying further spread.
He'll need to repeat PSA, at intervals of 1month, and 3 months.
In case he develops bone pain, commonly low back pain, he may get a pain relieving medicine from his doctor.
You may contact me with biopsy report diagnosis. Wish him well.

Brief Answer:
Prostate surgery is never done without biopsy report in suspected cancer.

Detailed Answer:
Hi again.
As high PSA is suggestive of possibility of prostate cancer, a bone scan was done.
That scan is also suggestive of spread into bone.
The only definitive way to establish diagnosis of suspected cancer is a biopsy.
As that has already been done, get that report from the Urologist and discuss the implications of treatment options. Once you discuss the biopsy report with him, you're welcome to contact me for further clarifications and suggestions.
As of now, a prostate surgery is not to be considered, without biopsy report.
Prostate gland enlargement symptoms , has several medical treatment options.
Hope your doubts are cleared.
Wish him well.

Brief Answer:
Biopsy has confirmed cancer prostate.

Detailed Answer:
Hi again.
Well i get a feeling, your doctor is not communicating with you, otherwise these doubts would not have arisen.
The biopsy report has confirmed the diagnosis of prostate cancer with spread outside the gland in the local area, and is of a high grade.
The bone scan has confirmed spread outside the gland, into the bones at several sites. Thus the tumor is of a high stage.
A high GRADE, high STAGE, prostate cancer has in general a poor prognosis.
The question of prostate surgery does not arise.
Such people are usually given hormone therapy, to delay further spread.
Hope your doubts are now clear. wish you well.
