Acitrom dose needs to be decreased.

Hello!
I have gone through reports. Main problem lies in her INR report. Our target INR is between 2-3, but her INR is 5 so we need to change the dose.
She should be on reduced dose of Acitrom once a day at 6 pm in the evening instead of current doses. INR should be repeated after 1 week to see the change.
Next thing, sugar post meal is acceptable although on higher side. If her readings are in this range only then it's better to have Tab Diapride M1 forte instead of Diapride M1.
so you discuss with local doctor regarding this. Her uric acid is fine.
