How Should One Diagnose Abnormally Enlarged Kidney And Fibroid While Experiencing Stomach Pain For Over Three Weeks?

Question: Hi,
Here are the attached reports of the patient who is suffering from stomach pain since last 3 weeks. Doctor says problem in kidney and a fibroid over there.
Please suggest what is the problem and what should be the best treatment

Hi,

From a PET-CT scan, it is difficult to give a diagnosis. If the patient is in good physical shape, I suggest a CT- or ultrasound-guided biopsy of the mass. It can be related to infection or cancer, and a biopsy will provide the diagnosis.

Hi,

Thanks for the response.

What about Kidney which is having severe problem and is not normal in size ?
Is the mass impacting the kidney completely ?

Doctor is saying that to do the biopsy they have to do the Operation which will take 2-3 hours .

The right kidney is small

Hi,

It is the right kidney that is relatively small. This is consistent with some degree of kidney disease. It is difficult from the reports to say whether the mass is completely obstructing flow from the kidney, but since there is catheter in place to drain the kidney, it is all OK. A biopsy would be the next step, providing that the patient is fit for treatment of the mass, i.e.. if a diagnosis will have a therapeutic consequence.

Please find attached the PCN kidney Cratenine report. Request you to kindly suggest if the kidney looks normal. In case the kidney functioning is restricted, please suggest if the kidney is recoverable or is in a state of complete damage where it cannot be rectified.

I cannot see a creatinine report.

Hi,

I cannot see a report on kidney function or an image of the right kidney - only the PCN pyelography. The best way to determine kidney function is by a renography, where the function of both kidneys can be assessed. If the obstruction is complete (that is difficult to say) and it has been so for weeks, there probably is no kidney function. But a renography can provide the answer to this.

Hi,

From the urine analysis I see that the excretion of protein is high, and the excretion of creatinine is low. This finding indicates a reduced kidney function, but it does not say whether one kidney is OK and one is sick, of whether both kidneys are sick. Here, the renography is the best diagnostic tool.
