Question : Dear Dr My details are 41 yrs/177 cms/79.5kg. I am having diabetes and BP for last 5 years. Ultrasound exam report in 2010 says moderate fatty infiltration of liver and mild splenomegaly. SGOT/SGPT levels were 39/68 in 2010, 55/99 in 2012 and now in 2014 is 55/107 . I am required to take a medical test for purpose of employment in middle east. Request to kindly advise on whether the level can brought to the normal ranges and if so if there is a possibility to do it before taking the medical..

Brief Answer: Hi Detailed Answer: I am doctor working in Liver and Biliary unit. I have read your query. I noted that you are overweight and may have abdominal deposition of fat. You may be leading a sedentary life. I presume you take medicines for blood sugar and blood pressure. The medicines can raise the liver enzymes so this may be medication induced as well. With the chronic medical conditions we commonly see raised SGPT and sGOT levels. This is because of fatty deposition in the liver cells and the function of liver will be compensated over a period of time. However it is not usual to see a spleen enlargement. So I suggest you to get evaluated thoroughly about it. As far as your query is concerned I am sorry to say that there is no medicine or procedure that can reduce the levels of SGOT or SGPT immediately before your scheduled medical test for your occupation in middle east. As far my expertise TLM (Total life style modification) is the standard care here along with control of blood sugar, obesity, blood pressure and fat in the blood.

Thank you for your reply. What could be the possible reason for the enlargement of spleen. Is it related to diabetes or i had jaundice hep a around 18 yrs back. Could it be caused by this or any other reason

Brief Answer: Mild splenomegaly has multiple reasons Detailed Answer: We will not be able to ascertain the definite reason in your case. A previous history of jaundice could be reason. Spleen enlarges if there is destruction blood cells more than its capacity to digest. It would be really tough to understand without investigating further. Sometimes liver dysfunction is also reason for splenic enlargement.
