Brief Answer:
Diabetes

Detailed Answer:
Sorry to note that you are unable to achieve good glucose control with metformin alone.

Discuss with your endocrinologist if you are a candidate for other medications such as
Januvia or Onglyza or Tradjenta
Invokana or Farxiga or Jardiance
Sulfonylurea

These are examples of medications in different categories which can assist you in lowering your glucoses.

In order to assess which one would be best suited for you, a physical examination followed by several blood tests such as

CBC
CMP
HbA1c
Lipids
Urine Albumin to Creatinine ratio
TSH

Obtaining a formal one-on-one consultation with a dietitian will also help, as will participating in DSME (Diabetes Self Management Education) programs
