Question: I have a 41 year old son who is in rehab at TLC (West Park, NY), second phase. They focus on the spiritual side of healing. They do not offer what I hear Dr. XXXXXXX refer to as dual diagnostic treatment. My son has been there since last May and I feel he needs additional treatment to remove his anger, build his self respect and prepare him to return to society. He is now repeating that he wants to leave TLC, but does realize he needs additional care. I need recommendations that will work on mental side of his life. I would greatly appreciate any information that you can share.

I can understand the stress you must be through regarding your sons health.
As I can understand your question is
1) regarding further treatment options for your his condition, for which he has been in rehab since last May.
2) he is been suggested for dual diagnosis by treating doctor.

Here it is important to asses the improvemt in his mental condition, his personal, social functioning ability before considering his discharge from rehab, so as to prevent relapse of his condition.
Further treatment plan depends on his diagnosed condition, for which he can be advised medications that will continue for long and psychotherapy session such as cognitive behaviour therapy, etc. on regular basis.
Depending on his response and improvement we can then gradually restore his daily functioning via graded exposure to life situations and work.

Here I need to know his problem and details of his symptoms, with duration and Treatment taken.

This is a long story/history, but he has abused drugs on & off since High School. He has had great clean periods with good jobs. Owns his own house, which I have had to make the payments for years. He got married in 2006 and she abandoned the house in July of 2013. I am working with an attorney to finalize the divorce. He has a daughter, 12 years old, from a previous relationship who is very close to my husband & I. We have her with us a lot. XXXXXXX was using/abusing Heroin at the end. He was stealing, removed the siding from his home for cash, let a drug dealer move into his home in exchange for heroin. I had to hire a private investigator and an attorney to get him evicted. XXXXXXX was in rehab during this time. The Drug Dealer destroyed the home, etc. XXXXXXX has been clean since XXXXXXX 2014. I spoke to him this morning at TLC to wish him Happy Birthday. I asked him to talk to his Life coach about a possible transfer and he said that he had gotten fired but didn't know why. The RA that XXXXXXX had when he was in Phase one, left TLC is is back on drugs. This was suppose to be his role model.
You asked about medications. When XXXXXXX left Arrowhead (Detox) (Toledo, Ohio), he was given 3 prescriptions, but I drove him immediately to TLC and they would not allow any medication or smoking. As I mentioned, there is not a physician on staff and everything is spiritually based.
TLC has several businesses that they transport the men to on a daily basis. They do not get paid for this work. The money goes back to TLC. They are not teaching them how to mange their lives when they get back into society. XXXXXXX feels like he like a servant to the program.
I have visited several times and I also question certain things that go on.
I need to find a suitable place to have him continue his treatment. I don't want to go backwards and end up losing my son.

It is very evident from your information, that your son is suffering from drug addiction problem, however there is no mention of any other psychiatric illness or medication by you(unlikely) ;and that he has had disturbed personal, social and occupational life.
In current scenario it's very important to asses his mental status, which will solely determine his future treatment course. Considering his disturbed personal life, a co occurring psychiatric illness such as personality disorder, impulsive disorder is very likely and needs psychiatric expertise.
I really don't have much of information regarding the professional expertise available at the TLC, but your son surely needs to be shown to a psychiatrist, he may require medications along with behaviour therapy sessions.
I will be better if you can discuss these and have an opinion of a Local psychiatrist.

Thank-you for your input! I am working with XXXXXX's younger brother to get him to a different facility. His anger regarding treatment at TLC is getting worse by the day.

Hi,
I think that's should be a better option to go for, as showing him to an expert psychiatrist and to a different facility may work.
Cognitive behavioural sessions are very much advisable in him, and would require an expert psychiatrist. It will not only help to break his denial, but also would ensure better treatment compliance.
