How To Keep HbA1c Level Under Control?

Question: Hi my age is 45 ( male), BMI 24. In my annual scan I got HbA1c 6.3%, though no previous alert on sugar.
Amylase 170.6 U/L and Lipase 63.2U/l. Blood urea, Creatinine, calcium , phosphase and thyroid are good.
Haemoglobin is 16.1. SGPT 25 U/l, SGOT 24.9 U/l and GGT 34.3. BP at end of full day of work is 130/90. I do not exercise more than twice in a week with desk job and travel.

Do I have to take a)Diabetes medicine and b)Blood pressure medicine ? I have already started watch on food, exercise 20 mts daily

Welcome to HCM. I am Dr Anshul Varshney. I understand your concern.
I have reviewed your query and reports.
With this HbA1c level I just suggest dietary modifications that include sugar free, low carbohydrate and low fat diet and regular exercises.
If you increase your exercise time to 45 minutes a day and that too jogging or brisk walking you would be able to control your blood sugars on this only.
This can't be ascertained that you would require the medications in future or not but with current scenario you don't require any medicine for diabetes.
Many doctors prefer to add Metformin at such levels but that practise is not supported strongly by clinical trials.
You have not mentioned your Blood Pressure readings. Tell me that so that I can guide you regarding that.
For diabetes just Follow this and get HbA1c and Fasting Lipid Profile repeated after 3 months and review back with me.

My Blood pressure at start of day in 120-80.

After full day of work and 2 hrs driving in evening it goes upto 130/132 and 87/90.

But after settling down at home in a couple of hours recedes back.

About 6 years back I used to take amtas 2.5 - 1 daily as precaution as have family history of BP. My physician wanted me to be 115/70 instead.

I had lichenplanus of hair in 2008 mid ( hair roots getting destroyed) . One of the possible reasons for that is taking BP medicine for long.

But after taking for say 2-3 years, got checked and without medicine also I was around 80-85 so have left taking Amtas from last 5 yes.

In last 2 years I have taken s atrenous travelling filed job and my exercise levels had gone down to almost nil and hence got weight increase from 66 to 69 and waist from 35 to 37.5. All this has reflected in HbA1c.

Do I restart the BP medicine now ? What would be the lowest start point of dose in case of yes ?

I also have another question - Is Amylase reading of 170.6 significant ? As in attachment.

Dear XXXXXXX

There is no indication to take BP medication at all in your case.
Your BP has been absolutely normal. No treatment is indicated. If you want to read you can read the JNC 8 guidelines for Hypertension. You will find that you were taking your BP Medicines unnecessarily.
Now yes, weight gain is definitely associated with increase in HbA1c levels which if not controlled would cause dyslipidemia and increase in BP.
So again you just need moderate weight loss, daily exercise and dietary modifications.

Now about your amylase levels , yes it is slightly high. Most common are two causes
1. Alcohol consumption
2. Gall Bladder stone induced sub acute pancreatitis.
3. Medicines.
So if you consume alcohol you have to quit it.
You will need an ultrasound Abdomen done if you have any jaundice or pain abdomen.

Stop all unnecessary medicines.
Believe in diet and exercise at this stage.
If still you want you can consume fenugreek seeds for better glycemic control.
