Question: Hi, I don't know if you remember me, but a month ago I spoke with you and was seeking counseling for BPAD1 and BPD. I ended up leaving because I didn't want to believe that I had those diagnoses. I still don't want to believe it, but things have gotten worse since I left, so I felt that I needed to come back. I wish I wouldn't have to deal with these for the rest of my life. I feel like I will never progress, and I will just always be miserable. Is there a chance that I may not really have BPAD1 and BPD?

Hello Loreal. I do remember you quite well. I'm glad you're back, as I think there are things I can offer you.

I have reviewed your attached records. I do sincerely believe you have BPD based on the information I have. BPAD1 is less clear to me, though it is a strong possibility.

I know these are upsetting diagnoses to receive, but that doesn't change the fact that you are having very serious symptoms, and the good news is that we have treatments for these things that can help you feel better. You will not always feel miserable -- but you do have to engage in treatment.

The treatment that is most important for you is dialectical behavior therapy (DBT). I would be willing to perform this with you via my private link. This is a treatment proven to help people with BPD.

Medications can also be helpful. I think lithium is a decent choice, though I would probably prefer Lamictal for you, as the side effect profile is better.

If you engage in therapy, your symptoms can and will improve. I do strongly believe you have BPD at least, and likely BPD and BPAD1. However, with treatment, your symptoms will get better. You will not be miserable forever.

Assuming I just have BPD, will DBT cure me? I guess my biggest fear is that I will have to deal with the way I currently think and feel for the rest of my life. I feel like I'm holding myself back. Even though I've been exposed to mindfulness, I find it hard to practice. I don't want to live in the now, because my now sucks.

BPD is a chronic illness, in the same vein as hypertension or diabetes.

DBT is a therapy that will help you develop skills to make this illness much more manageable, reduce symptoms, and vastly improve your quality of life.

BPD, like diabetes or high blood pressure, cannot be cured, but can be managed adequately such that you can live a much more normal life.

DBT is the treatment of choice for BPD and is very effective.
