Question : Hi. OK, I seem to have chronic tachycardia. I seem to have had it for a long time (how long, unknown); even before I took my boatload of meds! I took metoprololo for it which worked pretty well; but I have so many meds I wanted to see if I can get off it. So, my doctor and I agreed to have me wear a Holter monitor for 30 days, weaning me off the metoprololo. Without this med, I fe?t things I felt less of with the metropolol. I very often feel out of breath doing little (slow or moderate walking, talking, or just dressing (!) . I once in a great while get chest pain. Am I making a mountain out of a molehill or should I be at all concerned about this (soMy pulse is almost always tachy, when at the office or if I take it)? If I should be concerned, what are the options of treatment do I have?

Detailed Answer:
Hello!

Welcome and thank you for asking on HCM!

Regarding your concern, I would like to have some more info about your heart rhythm:
-What is your mean heart rate and the XXXXXXX heart rate?
-Which doses of metoprolol did you use? Do you feel better without taking it?
- Could you please upload your ambulatory ECG reports for me to review?

I would recommend performing some tests to try to find the possible cause of your tachycardia:

- a cardiac ultrasound besides the ambulatory 24-48 hours ECG monitoring
- blood lab tests (complete blood count, fasting glucose, blood electrolytes, thyroid hormone levels, cortisol plasma levels, kidney and liver function tests).

A treadmill stress test following with other tests ( stress echo, cardiac enzymes, etc) may be needed to exclude possible cardiac ischemia as a cause of your chest pain and cardiac arrhythmia.

You should know that anxiety could also contribute in a heart rate.

Hope to have been helpful!

Feel free to ask any other questions whenever you need!

Best regards!
