I can understand your concern. You are suffering from severe lower back pain, most severe after using the restroom. Your most investigations were normal. As you mentioned that similar episode occurred in 2009, which was treated by antibiotics. I am suspecting there was some infection at that time. Nature of infection like urinary tract or gastrointestinal or spinal is not clear.
Please let me know the following information so that I can make some differential diagnosis
- What is the exact location, midline lower back or right/left paraspinal region?
- Does this pain occur with urination or defecation? If occurring with urine, does it radiate from back to front, in pubic region? If occurring with defecation, does it localize to anal opening?
- Is there any history of burning during urine, increased frequency, hesitancy, fever, foul smelling urine?
- Is there any history of indigestion, loose stools or constipation, cramps pain abdomen?
- What is the nature of pain? cramp-like, sharp-burning type, current like with radiation to lower limbs?
- Do you have any history of lumbar disc disease?
- Is there any history of vaginal discharge, pain, and lower back pain along with it, fever?
I am asking these questions because severe lower back pain can occur due to:
- Lumbar disc disease or spinal pathology
- Urinary tract disease like stone/infection
- Lowe intestinal disease like colitis/anal fissure/anal fistula
- Some vaginal disease like pelvic inflammatory disease or vaginitis.
