Question : I am 75 years old, with diabetes type 2 for the last 10 years. I'm on glucovance, onglyza and lantus (60 units/day), among several other medications yo control blood pressure and cholesterol. Overall my health is good; my A1C is 72. I am uncircumcised. For the last year, my foreskin has been itchy and progressively tightening to the point that it is not possible to pull it back over the glans. I have been careful to clean myself and am treating the itching with topical application of OTC Cortaid. While the itching is mostly tolerable, urinating has become a problem due to lack of directional control. I am not sexually active. How do I control these problems?

I have gone carefully through your concern.
The symptom you described is suggestive for local infection that may precipitate from elevated blood sugar.
First you need to keep Diabetes a bit more controlled (depending on daily measurments your physician may add rapid acting insulin, if postprandial glucose lecels are high).
You need also to consult an urologist for oral antibiotics for the chronic infection of the glans penis. You need of course a urine sample examination to rule out urinary infection.
Chronic infections may be also a cause of uncontrolled blood sugar, making it a difficult vicious circle.
So, blood sugar and infections need to be treated at the same time.
