Question : Hey doctor,
I am a 20 year old male and I’ve had this burning sensation when I urinate along with this dribbling in my pants shortly after I finish urinating for the last 6 months now.
I’ve had urine problems since I was a kid I always used to hold on for very long periods of time before going and it was more of a mental thing I had to go over and I think the damage from that has caused the dribbling. In terms of the burning though I am unsure what can cause that? In the last 6 months I have had a urinary tract ultrasound, testicle ultrasound and 3 urine tests that where cultured and tested for bacteria and yeast. My gp things its nothing but would it be worth seeing a specialist?
What else can mimic these symptoms cools if possible be a form of prostate/testicle cancer? Im just a little worried since my friend was recently diagnosed with lymphoma cancer at my age from pestering symptoms of a cough for the last year. In terms of the pain it’s not really bad but I find it stings more if I have masterbate and that’ll last a couple of days.
What do you think this and advice I do?

I also find masterbating aggravates tbe burning sensation and the burn comes on at the very start of peeing having a lot of water seems to help.

Based on the facts that you have posted burning urination and post void drilling that you have since childhood could be due to Congenital Bladder Neck Obstruction (BNO)which needs to be confirmed by relevant tests
.
Consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Urodyanamic study of bladder
4) Endoscopic Examination of bladder (Cystoscopy)
Further treatment will depend upon result of these tests and final diagnosis.
As regards pain while passing urine which gets aggravated after masturbation is common and normal because of following two factors .
1)During erection and ejaculation urethra is congested due to increased blood flow which remain for few hours
2) During ejaculation a small volume of thick semen is adhered to urethral wall which obstructs the free flow of urine giving rise to pain while passing urine when one passes urine for the first time after masturbation .
This is normal and natural and noting to be worried about .
Ensure to drink more water .

I’ve already done those three tests that you have mentioned besides the cystoscopy I did the the third one when I was younger and all the others quite recently would it really be necessary for someone my age based on the other tests being clear? The burning pain seems to be the biggest concern for me but it’s directly related to masterbtion if I don’t masterbate for a week or two the burning pain goes away but as soon as I start again it does even after the first urination.

I also remember doing an x-ray as a kid for it as well when I first starting having the problems which turned out to be clear as well.

I’m just a little scared of endoscopy as I’ve had damage done in the past from then when I did an upper gi endoscopy. Based on the recent ultrasound and urine tests being clear what are the chance of it being BNO if whatever I have isn’t treated could it get worse I’ve had it for a while now and it hasn’t really gotten worse?

Could overdoing masterbating be the cause of burning? I’ve been doing it 1-2 times a day for quite a long time would that may have caused some damage maybe even the cause of the inconterience?

My dad had a cystoscopy done and he has had pain ever since from it I’ve really only heard bad things about it and I’d rather avoid it my age if it’s not super necessary.

Can the cause of these symptoms be related to cancer or are likely to be what would Red flag symptoms be for it?
