Question: I am a 27 year old female with a chronic stomach pain. I have add this pain for approximately three years which is constant during the day and often wakes me from my sleep. The pain is in the upper center of my stomach. More recently it has been discovered there is an indentation in the area of the pain. Some of the tests already performed are as follows: 2 colonoscopies, 3 endoscopes, 1 capsule endoscopy, 1 gastric emptying scan, 1 HIDA scan, multiple ultra sounds, allergy tests, cat scans all with negative results. The results of the capsule endoscopy will be received on Monday, September 10. I've been on steroids, bentyl and several other medications all of which did not offer any ease. A little sludge was found in the gallbladder but the doctors did not feel it was enough to require surgery. What are your thoughts regarding a diagnosis, tests, medicine, specialists or questions to pose to my physician . I will await your response.

As most of the tests related to gut are normal i think we should move toward sone common things which presents with the rare symptom.
Firstly i would suggest you to go for 2 tests of cardiology namely 2d echo and tread mill test.Some times coronary artery disease can have the same presentation.
Second can be irritable bowel.In IBS there is some problem in the functional ability of the gut that leads to the symptoms of irritable bowel syndrome. The structure of the intestines is normal, only functional abnormality occurs in this condition.So frankly speaking (IBS) represents a functional disorder of gastrointestinal tract without the presence of an anatomic defect.So all tests would be normal however patient would be symptomatic.
I would like you to start on-
Dicyclomine 30 mg and Mebeverine combination.Both are anti spasmodics.

Follow up after a week.

Thanks for a prompt response. I already have taken dicyclomine which was ineffective. IBS was ruled our by my doctor because none of the typical medication gave me any ease. Suggestions?

Hello again,
Dicyclomine is just the initial first line drug antispasmodic and is not specific for IBS.
Before ruling out IBS as a cause-
Clidinium bromide is an anticholinergic (specifically a muscarinic antagonist) drug. It may help symptoms of frequent stools.

Chlordiazepoxide works by increasing the action of GABA, a chemical messenger which suppresses the abnormal and excessive activity of the nerve cells in the brain.
These three should be given in combination.They are all antispasmodics and would help with spasms in intestines.Get a prescription for them and try them initially for 3 weeks.
