Question : I am a 44 year old female- I was just diagnosed with an anal fissure. The burning/stinging pain has lessened but I am still experiencing rectal pressure/tightness. At times it feels like I need to have a bowel movement. Is this feeling of pressure common with anal fissures almost feeling like I need to run off to the restroom? How can I speed up healing?

Detailed Answer:
Hi,
Thanks for posting your query.
I am Dr.R.K and I am pleased to assist you.
You have mentioned two symptoms -
1. Tightness at the anal verge
2. Urgency - which makes to run to the rest room
The tightness is due to spasm of the anal sphincter. The following may be done for relief-
1. Sitz bath for 10 minutes twice a day
2. Apply topical diltiazem at the anal verge which will relieve the spasm
Urgency is not a symptom of anal fissure. Urgency is a symptom of rectal lesion. If symptoms persist sigmoidoscopy may be done to look in to the rectum to see if it is inflamed.
Regards,
Dr.R.K.

Hi,
No, internal hemorrhoids do not cause urgency.
But a grade III and IV hemorrhoids can cause a sense of pressure at the anus. In these cases, you can feel the hemorrhoids as a lump at the anal verge.
Yes, anal fissure takes a long time to heal.
