Question : I am concerned about my bp readings tonight. I am on 80 mg telmisartin and 10 mg Amlodipine. That keeps me at about 147/97. Tonight my first reading was 197/107. Waited 5 mins and it was unchanged. Waited 5 mins and changed arms and it was 188/109. Waited 10 mins and it was 166/97. So it seems to be coming down but it has never been that high before. I live in XXXXXXX and am staying in WY for about 6 mos at about 7500 ft elev. just need to know if I need to go to an ER. I have an appt with a dr on 12/4 to investigate what I believe to be angina.

I understand your concern and would explain that it seems that your current therapy is not very effective in controlling blood pressure values, considering your high blood pressure values.

Coming to this point, I would recommend increasing the dose of amlodipine to 20 mg daily and closely monitor your blood pressure.

It is also necessary to reduce salt and caffeine intake.

If your blood pressure values are still high after increasing the dose of amlodipine, another option would be to add another antihypertensive drug to your therapy ( like Hydrochlorothiazide or moxonidine).

There is no need to go to the ER if your blood pressure values drop with the increase in the dose of amlodipine.

Otherwise, some tests may be needed to investigate for secondary causes of high blood pressure:

- thyroid hormone levels for thyroid dysfunction
- kidney and liver function tests
- blood electrolytes
- fasting glucose.

Although you should know that the changes in the atmospheric pressure( changing living place) may have some implication in the changes in blood pressure values.
