Question: I am having cracking foreskin problems. Painful and hard to keep clean. Clears up and returns back again. I wear a diaper 24/7 to prevent an accident. Don't know if that is the problem. When I get foreskin back the head of my penis has this cheesy look. Having bleeding problem lately trying to keep clean.

You can have balanitis problem according to history. It is the inflammation of foreskin and glans penis. So yes it is the problem related to foreskin.

Foreskin is sensitive area and can get easily inflammed if foreskin and underlying smegma not cleaned properly.
You are advisable to clean it with lukewarm water.
Don't apply soap for few days.
Keep penis area clean and dry after bathing.
Take healthy fruits and vegetables more.

It seems to be fungal balanitis. For that antifungal ointment or cream can be prescribed. If inflammation more then mild steroid cream also can be prescribed. You can Consult dermatologist for examination of genitals and get prescription accordingly.
