Question : I am having erectile issues and I am only 27 years old. I am not overweight, unhealthy, etc. 4 months ago I found out I had phimosis and my doctor prescribed triamcinolone cream to help with the stretching. I used the cream for 5 days and my foreskin retracts now while flaccid but after using the cream every day for 5 days I woke up one day with no morning erection and on my way to work I had felt my penis feeling numb and dead and having no sensation. I freaked out and when I came home I was having a panic attack since I couldn't get an erection. I admit, this may have been due to my anxiety since I was super stressed about my numb penis. In these 4 months that this issue was occurring I only get morning wood about 3-5 days a week instead of daily (like I used to). I have seen 4 doctors for different opinions and all 4 of them told me to mediate and see a counselor since they say it is due to my anxiety which I do not accept since if it really was due to anxiety I would still get morning erections since it is unconscious. I have tried to have sex as much as I could these past 4 months not necessarily because I am horny but because I curious to see if I am still able to get erections from being aroused. I was able to get an erection every single time but I do not remember if my erections were as rigid and hard as before. I honestly do not remember what my erections felt like before. Also, I am having a tougher time masturbating to pornography. Plus, with my morning erections, when i do get them they don't last as long. As soon as I wake up I feel my erection going away whereas before I would still have the erection when I would got to the bathroom and urinate. The only things my doctors did was take a blood test to check my testosterone levels which came back fine. None of them are offering a test to check the blood flow to my penis, they aren't offering an ultrasound to check my penile arteries and veins or an MRI for my penile nerves. All they do is advise me to see a counselor and prescribe me viagra which I refuse to take. I find it very insulting that my doctors are basically syaing that for 4 months I've been having these erection problems due to anxiety and they just want a "quick temporary fix" by offering oral medication and not trying to understand the root of the problem. All 4 of them say the same thing...that i am too young to have erection issues and that this is all in my head. Is there no chance at all that a 20-30 year old would have physical impotence? I am not making these symptoms up. If I could go back to how my erections used to be 4 months ago I would in a heartbeat but I am very disappointed in these doctors who basically are trying to convince me that nothing is wrong with me but haven't even done any kind of tests other than a basic blood test. Is it possible that the triamcinolone cream caused this? Is it possible that my nerves and arteries in my penis are damaged? The only reason I think this is because one day I forced my foreskin back while I was erect and tried to masturbate but it felt very uncomfortable and very tight around the head of the penis. Would that have caused any damage? I also read online that porn addiction may cause all these issues i am having. Is that true? Please somebody help me because I am too young to be doing through this. I have been thinking about this for the 4 months this has been happening. Sometimes I feel like my erections are going back to normal but then it crashes again. I am just really concerned that by the time I am 30 I will not have erections at all. Please advise.

Brief Answer:
You have Erectile Dysfunction which is Psychogenic in nature

Detailed Answer:
hello
Thanks for query .

Based on the facts that you have stated the Erectile Dysfunction that you are facing since last four months is definitely Psychogenic in nature and all the four doctors have rightly diagnosed your issue to be due to anxiety.

There are no diseases that can damage your nerves ,arterial blood flow to erectile tissue without any evidence or any other symptoms .

Erection is very complex process which involves psychological stimuli to initiate the chemical and neuronal impulses to relax walls of arterioles in erectile tissue to increase blood flow in erectile tissue to get hard erection hence any disturbance in psychological process (Anxiety ) is the primary factors that causes Erectile Dysfunction in young adults.

You need to consult qualified Psychiatrist for counselling and take treatment if required .

Following general measures will help you to boost up your confidence and getting good erection to have enjoyable sex .
1) Practice regular exercise for 45 minutes followed by meditation for 1/2 an hour in the morning.
2) Take high protein diet rich in vegetables and fruits and Vitamin A,C,D,E.and Zinc
3)Take anti oxidants like Almonds 5-6 everyday..
4) Avoid alcohol and smoking..

Taking Sildenafil (Viagra)as on demand will help to get good hard sustainable erection and to have enjoyable sex.

Though it is harmless, excessive and frequent habitual masturbation does affect quality of erection and delays ejaculation.Try to reduce the frequency of masturbation by keeping yourself busy in activities like sports, reading ,social work etc so that you will not get free time to masturbate.

Hope I have answered your query ,please feel free to ask if you have more questions,I shall be happy to help you
Thanks and Regards.

Thank you doctor. What about the lack of morning erections? Would anxiety have an effect on that as well? Also, if I try not to masturbate or have sex for a month just to see if everything goes back to normal would that be okay? Or would my erections become weaker if I don't use them?

oh okay. I guess I will just try to meditate and distract myself from thinking about this.
If I were getting regular every day morning erections I would feel much better because then I would know that physically I am fine but just need to work on my mental health. Isn't the lack of morning wood a sign that something is very wrong with my erections and health? 27 years old is a little too young to have such a sudden change happen with erections right? I would assume it would have happened more gradually just like any other changes in the body that come with aging.

It is not necessary that one should get morning hood every day.

Detailed Answer:
Hello
Thanks for follow up.

It is natural and common to get morning erection at young age but it is not a rule that every one would get it neither not getting morning erection every day should be considered as abnormal or something is wrong with sexual or reproductive system .

Your basic problem is anxiety you should try to allay it ,

Thank you Doctor,

This is reassuring. I will try meditation for and try to see a counselor once a week if my insurance approves it. Also, how long does anxiety effect the body? There are some days where I do not feel like I have anxiety. I do think about my erections every day but there are moments where I am calm and happy and optimistic but I still feel like my erections aren't has hard or rigid as they used to be. Wouldn't anxiety only effect me at the exact moment that I have anxiety? Or would the effects of my anxiety (pertaining to my erections) stay in my body until I try and get rid of my anxiety instead of just trying to ignore it.

is there a cure for ED that might occur soon? Not treatments like oral medication or those implant surgeries but actual permanent cure to have hard erections naturally again? Generally speaking.

Hi Doctor,

What about the fact that my erections don't last as long as they used to? In the mornings, when I get morning erections they tend to disappear before I even get out of bed and if I am sexually aroused and the arousal goes away then I feel my erection disappear quickly whereas before I would still be erect for a while. I've done research and think that maybe I have venous leak. Is this possible? Again, I don't have any chronic illness such as diabetes or peyronie's disease but would something else have caused the veins in my penis to leak? Sorry, if I am asking too many questions. I am not only annoyed with this situation but since I am trying not to masturbate or have sex for a while to see if my erections become stronger I am becoming very frustrated since I am depriving myself of sexual pleasure and it is making me even more annoyed.

Thank you, doctor.
I have had anxiety since late 2012 from what I remember.
Not only health anxiety but anxiety in general.
I will try to meditate at first and if I need to see a counselor to help the process more then I will see if I could find me.

I will also try to keep myself busy and distracted as much as possible so I can go back to how I was before.

I really appreciate all of your help and for being patient and answering my questions.

I know it is not easy dealing with patients with health anxiety so I just want to say I appreciate you and all you have done.

Hi doctor,

I just have a few more questions to better understand my situation. Would premature ejaculation be a symptom of physical erectile dysfunction? Ever since I have had these erection issues I've also been ejaculating very early (less than a minute) sometimes. Other times, my ejaculation time is normal. The PE happens while i masturbate as well so this is not only while i am having sex with somebody.

Brief Answer:
Anxiety is the primary cause for early or premature ejaculation .

Detailed Answer:
Hello
Thanks for writing again .
Early ejaculation that you get while masturbating is due to excessive penile stimulation of the penis by the firm grip of hand over penis .

Primary factor in either Early ejaculation or Premature ejaculation is what is called as Performance anxiety .which needs to be tackled on the lines which I have already explained in my earlier reply .

Thanks and Regards.

Hmmm...I don’t really grip my penis too tight while masturbating. I’ve always gripped it the same all my life but I’m having these issues only now...

Plus I don’t get performance anxiety. I thought you said my anxiety was more due to health anxiety since my erecrion problems don’t only occur during sex but also during masturbation and also morning erections/spontaneous erecrions.

Have there absolutely been no records of any male in their 20s having physical ED?

I also mentioned that in my first paragraph that I recently found out about my phimosis just this year and that did make me a little anxious as well. I thought about it a lot and was a little depressed by it. Would that have been the start to my anxiety problems regarding my penile functions?

is i gauranteed my erections will go back to how they were before all of this started if I treat my anxiety? I think my erectile health is more important to me than my mental health (if I had to choose 1)

Brief Answer:
Phimosis needs to be confirmed by clinical examination in person

Detailed Answer:
Hello
Thanks for follow up .

Phimosis whether it is partial or complete can be confirmed only on clinical examination by a qualified General Surgeon or Urologist .

Fully retractable foreskin along with sound mental health are the basic requirement to get good hard erection. You may consult qualified General surgeon or Urologist for clinical examination in person He would be the best judge to decide whether it requires any surgical intervention or not .

There are every possibility of your erectile function to return to normal after treating your anxiety .

Thanks and Regards.

and on a related topic. is there a reason for my phimosis? I used to get eczema/psoriasis scars on my penis when I was a teenager, would this have caused it never to retract? I ready that less than 1% of uncut men would have phimosis by the time they are 18. I can retract my skin while flaccid but can only retract over my glans when erect. Would a little bit more of stretching excersizes fix my phimosis? If my foreskin already strecthed this much it can't just stop at this point and not retracted nay further correct?

Doctor,

my boss won't allow me to go into work late once a week (which is what I need to do in order to go see a psychiatrist). Is there anything I could do on my own to get rid of the anxiety by myself without medication or a psychiatrist? I'm having another episode of a panic attack where I read online that a man who doesn't have physiological ED should get erections nightly. I haven't had a morning erection in the past 3 days and if I wake up tomorrow without an erection as well I will really be worried. I usually never went 4 straight days without a morning erection. Don't nocturnal penile tumescence indicate erectile health? I used to get them every single morning before March of this year.

I am done with my studies and the reason why I am thikning about it all the time is because it is/was a very important part of my life as it is for most people so the fact that everything is not functioning properly is frustrating. Also, I find it strange that as soon as I fixed my phimosis I got all these problems related to erections, morning erections, premature ejaculation, etc. I don't think it is very common at alll for men in their late 20's to barely cure phimosis so i don't know how much research or studies are done to the effects of curing phimosis or using the steroid cream.

thank you. I have my first session with the psychiatrist on Friday so hopefully everything goes well!

So basically, the reason why none of the 4 doctors I saw were willing to do an ultrasound or anything on my penis is because it is scientifically impossible for me to have blood flow issues in the penis or to have any type of venous leak without any underlying condition? Is it possible to have type 2 diabetes with the only symptom being ED? Would having diabetes even effect erections in your 20s? Also, what would the symptoms of venous leak be?

I am just trying to use all my questions before I get to the limit.

ED may not be a presenting symptom in type 2 Diabetes .

Detailed Answer:
Hello
Thanks for follow up .

Type 2 Diabetes usually onset after the age of 40-45 years and ED rarely a presenting symptom.

Partial Erection not hard enough to have penetrative sex is symptom of venous leak.

I Am Having Erectile Issues And I Am Only 27

I am having erectile issues and I am only 27 years old.
