Brief Answer:
few possible causes

Detailed Answer:
Hi and welcome.

In most cases, this is just temporary pain due to pulled muscle or pinched nerve in the scar area.
Second, it may be beginning of hernia and if you feel bulge or pain during cough you should do an ultrasound to rule this out.

Third, it may be late postoperative infection and it would manifest with redness, discharge, and increasing pain.

I suggest to wait and see in what direction will it go. You can take nonsteroid painkillers and apply cold compresses to relieve pain.

Wish you good health.
Regards
