2. Last sex exposure was 2 weeks back and yes i wore a condom.
3. A similar rash happened 5 months back but it was apparently infected hair follicle which was treated with betadine.

Thanks for the follow up.
Even this rash appears to be infected hair follicle.
It's uncommon site for herpes.
Apply ointment mupirocin twice daily for a week.
This should help you.
