Question : hello I am in need of a gynecologist. I tested positive for pregnancy on 4/23/2020. I had my first transvaginal us on 5/22 I was told I had what looked like a healthy gestational sac and yolk but had no fetal pole visible. I went back today 5/29/2020 to see the progression. There is a fetal pole now, however they could not see a heartbeat and the Dr. was concerned. I am 39 years old, I am having pregnancy symptoms that are progressively getting worse especially nausea. I also have a tilted back uterus and I’m not sure if this has anything to do with my results so far. I’m not exactly sure of my lmp as I started spotting on 3/25 but did not bleed until 4/1. My pattern had been that I get my period or start spotting approx 3-5 days sooner than my previous period, I hope this helps. I am trying to remain hopeful but this is all very confusing. Also no cramping ( unless I have gas) and no bleeding, although I have wiped two times in the past two weeks and had a small amount of brown blood, but only upon wiping. Thank you for your time.

Thanks for trusting us about your healthcare.
I have gone through your history and understood your concern.

In my opinion, as your last period date is not very clear, maybe your conception is delayed that is why we are unable to see a heartbeat in your ultrasound (because it occurs between 5-7weeks).

Your doctor worried about it because it is almost 2months from your last period date.
But in my opinion repeat ultrasound needed after 7-10 days for fetal cardiac activity.

Get your blood beta hCG levels done also.

Brown color spotting could be due to hormonal imbalance caused by pregnancy, no need to worry about it as there is no pain, etc.
