Question : I am seeking medical advice regarding some liquid on the side of one of my testicles. I have had an ultrasound, as well as an MRI, which did not raise any concerns with the doctors and did not indicate any serious matter, such as cancer. I went to an infectious disease specialist to see if it was infection related, but was determined not to be. However, shortly after seeing the specialist I was not able to follow up with the doctor, as he has been working outside of his practice with focus on the corona virus matter. The liquid has been around for approximately 18 months and sometimes it feels as though there is some water retention and it gets a little swollen, but then will go back down again. There is no associated pain, though I am trying to figure out what the next steps would be. I had some blood work done and everything came back perfect, all levels great and no indications of any type of illness. Therefore, I am confused as to what to do next.

Brief Answer:
Epididymal cyst, varicocele or hydrocele possible. need further details.

Detailed Answer:
Hello and welcome to Ask a doctor services.
I have read your query and here is my advise.

* As per my clinical experience, liquid on the side of testicle may be related
to various issues such as

- Epididymal cyst ( over superior dome of testicle )
- Varicocele ( feeling of bag of worms )
- Persistence of Tunica Vaginalis as Hydrocele
- Hydrocele of the spermatic cord

* Usual cases with above conditions do not reduce in size except that of
Varicocele which reduces on lying down.

* Another possibility apart from liquid content may be that of inguinal hernia
pushing inside the root of scrotum and giving feeling of gush of liquid
entering into the scrotum and going back again.

However, if you can provide following details , it would be better to discuss further in this matter.

1. Progress of the condition since beginning.
2. What are the maneuvers which make it prominent ?
3. By which way it becomes less visible or disappear ?
4. Do you notice any abnormal bulge when you cough or raise intra
abdomen pressure at the region of inguinal portion ?
5. Can you pick a picture when it is visible outside ?
6. Can you share USG, MRI reports here as an attachement ?

Awaiting for your response.
Regards.
