The articular cartilage in the medial compartment demonstrate moderate thinning and surface irregularity with small full-thickness fissures. The articular cartilage in the lateral compartment demonstrates heterogeneity and thinning with small full-thickness fissures.
There is a mildly complex horizontal tear of the posterior horn of the medial meniscus. The lateral meniscus also demonstrates a small horizontal tear posteriorly."

I have seen the attached MRI report. The scan suggests age related degenerative changes in the knee joint due to wear and tear. There is loss of cartilage thickness with surface irregularity suggestive of osteoarthritic changes.
The small tears in the menisci are also related to the degeneration and wear and tear. The subcortical cysts are also suggestive of degeneration of the bone cortex. All these can improve with medicines and some physiotherapy. IN case the pain is severe and uncontrollable with medicines and regular physiotherapy then joint replacement can be considered.

Surgery is the last option if all the other treatment options fail. Using knee brace will help in providing an extra support. You can remove the brace while resting or use an elastic knee brace for a better flexibility.
In addition to the anti inflammatory medicines you need to have joint lubricant supplements to decrease the pain.

The tears can heal with rest.

The tear in menisci is related to weakness of the menisci. It is not a healthy meniscus. Even if it is repaired surgically, it will be difficult to heal as the cartilage is also thin. It is better to let it heal on its own gradually. In case there is a persistent intense pain then it is better to consider replacement.
