Question: I ejaculated on Friday and blood came out. Since then my testicles have been feeling heavy every now and then. I’m on vacation overseas and won’t return until 9/10. Are these symptoms that can wait until I head back home on 9/10 or should I return home immediately?

Brief Answer:
It is generally not urgent and you may be fine to wait till to finish your vacations

Detailed Answer:
Hello and welcome to "Ask a Doctor" service,

I carefully read your query and understand your concern.

Most patients with hematospermia (blood when ejaculating) do recover spontaneously, so in your case, because this was a single episode you may continue to enjoy your vacations and go for a visit when you go back home.
If you notice any worsening or the bleeding or any swelling or pain in the testicles, more important conditions may be considered and a visit would be recommended soon.

Infection is a common cause in young patients and should be considered if patients are at increased risk for STD (recent unprotected intercourse).
Also, prolonged abstinence (not having intercourse for a prolonged time), excessive intercourse or masturbation, trauma, etc, may cause some bleeding.
Rarely, a tumor may be the cause of blood in the sperm.

To conclude:
- generally, there is no urgent cause of the bleeding in the sperm so you may enjoy your vacations and see your doctor when you go back home.
- especially the lack of pain makes it easier to just wait for a while till you have access to your doctor
- if you notice any worsening of the symptoms (bleeding is worse or you start to get pain, swelling or any other symptom) you may need to see your doctor.
- in this case, some tests may be conducted such as testicular ultrasound, urinalysis, sperm analysis (microscopy), etc.
- most of the cases do recover completely without any treatment, especially in young patients and the rest may be due to infections (STD), local trauma, long abstinence or exessive masturbation or intercourse.
- rarely, the blood can be due to the partner (if the partner is bleeding), or a tumor

I hope this answers your query.
