Further investigations are required.
Detailed Answer:
Hi Sir
Thank you for asking ask a doctor service.
I have gone through your query with diligence. Chronic granular pharyngitis is a benign condition and nothing to worry. It can be due to recurrent viral or bacterial infections or due to laryngopharyngeal reflux.
For this I would suggest proper antibiotic therapy.Proton pump inhibitors like Esomeprazole and anti emetic agents like domperidone should be taken for 4 to 6 weeks. Not irritant diet should be followed. Taking vitamin C regularly also advised. I would also suggest keeping mouth full of water morning after brushing and evening before going to sleep for 15 minutes and spit out.
But your ESR value is high plus there is weight loss also. It indicates some strong inflammatory process running in body. Sputum AFB test should be done to rule out TB. C reactive protein and auto immune profile should be tested. ASO titre also should be checked. These are blood tests you can carry out in most laboratories. Chest X ray also should be taken.
Please upload your blood reports if possible.
Let me know if anything not clear.
I am happy to help.
Thanks.

                                    Brief Answer:
Thyroid problems are ruled out.
Detailed Answer:
Welcome again.
Thank you for your follow-up query.
I have gone through your reports. It rules out problem with thyroid gland. But culture report shows klebsiella bacterial infection. Antibiotics should be started according to the sensitivity. From that Coprofloxacin can be started. High ESR can be a sign of acute infection also. Better to do sputum AFB to rule out tuberculosis, C reactive protein ASO titre and auto immune profile tests to rule out those
Thanks

                                    Hello sir
I have attached other reports...
AFB SPUTUM and CRP .
Also sir i wanted to check my esr again with a different lab because heard some wrong things about the previous lab.
Now esr is down
I am on 3rd day of my antibiotic..
Can esr change because of the use of antibiotics and it will be back to that value once i finish my antibiotic course..
or m actually getting cured??
Or there is a possibility that esr doesn't change this frequently , it is the error of any one lab??
Please ans. this sir.. this is very imp for me to know
                            I have attached other reports...
AFB SPUTUM and CRP .
Also sir i wanted to check my esr again with a different lab because heard some wrong things about the previous lab.
Now esr is down

                                    Brief Answer:
Decreasing ESR and Normal CRP indicates- responding to antibiotic.
Detailed Answer:
Welcome again.
Thank you for your follow-up query.
I have gone through your reports. Decreasing in ESR is a good sign and it indicates you are responding well to antibiotic. Your CRP is with in normal limits. So no need to worry. CRP level is a very good indicator for showing signs of infection or inflammation than ESR and it will rapidly change when infection get resolved also.
No need to test ESR again. It will gradually decrease to normal range. You are getting cured only. No need to worry at all.
Thanks.

                                    Brief Answer:
If completed antibiotic course then its ok.
Detailed Answer:
Welcome again.
Thank you for your follow-up query.
If you have completed the full course of Coprofloxacin then its enough to clear infection. Checking for bacteria is not necessary. Laryngopharyngeal reflux can still irritate your throat. So taking a proton pump inhibitor like Esomeprazole (Nexium) for 4 weeks on empty stomach early morning is also advised.
Taking a spoonful of honey morning and evening will give soothing for your throat also.
Vitamin C ( Limcee) you can take. Taking one third of 500 mg will be enough daily. Zincovit daily not required if you are taking a balanced diet. Steam inhalation is good to avoid dryness in throat. But excess steam inhalation is not recommended.
If still irritation is there then throat examination by ENT specialist should be done.
Thanks.
