Question: I got very sick May 2017 and have been sick on and off since then. I got different 3 utis and vaginitis which rested positive for strep A! All that and just general malaise. I recently took two ebv virus mono tests as I suspected this and they were positive. I will attach results. The IGM was negative but ebv early Antigen positive twice with increasing load . Is it possible I am still contagious? Is it possible I've had Momo since last May 2017 undiagnosed? Please help

Hi,

Sorry to her of your long standing sickness Few facts about Infectious mononucleosis are following .
1.Its extemely common and most of the people already do have it once .About 90 percent people do have it in early life .initially it may be asymptomatic or may produce fever sore throught mostly remit itself and then give life long immunity .

2.there is a possibilty that the virus may be lying in body sometimes.

3.IGM is most of the times positive in acute infection after 2 to 4 weeks , however uncommonly it can be negative .and in such cases we do the early antigen antibody only if suspicion is high .

4.Early antigen antibody positive most of the times an acute infection and that the virus is actively replicating the body ( and hence you may be contageous) .This becomes negative after 2 to 4 months but .So this means the j fection is recent one.Howe'ver in 20 percent Peopel it may be positive upto an year .So may be possible ( though not common ) that you might have an year back. But the rising titers indicate that the virus is replicating .

So possibly you could have the fection recently and proabbaly you may be contageous .It's mostly transmitted through saliva ( kissing disease )

Howe'ver as already mentioned the diease mostly has a self limiting course.

Thank you for thorough response. Is there any other disease that would cause Ebv early antigen
to replicate? It has doubled from my July to August tests. I took them 4 weeks apart.
Is it possible I had it last May 2017 and again recently?
Would you classify my case as Chronic Eve/Mono?
How can I get treatment? It doesn't seem to go away!

Hi Good day...I AM really sorry for getting late with the reply , infact had a very busy day at hospital today.
The EA antibodies are reasonably specific ( about 90 % in some studies ) so we assume they ar less likely to be because of other causes .
These antibody generally clear away in few weeks but in upto 20 percent persists for an year .So we can't say it's chronic if they persists .However its presence indicates the virus is there at the moment .
Yes this may be possible that you might have it an year back and it has re -activated or has gone chronic which is not common.Howe'ver we are nt sure for it.

In pt with EM , prolonged period of malaise and fatigue has been commonly reported in people even after the infection is clear.

Whatever the infection is benign one mostly , and does not need specific treatment. You just take care , take more fluid , avoid taking amoxicillin, Resr I would advise there is no need for more tension .Just give time to your body to clear it.
