I Had A Lap For Endometriosis In Early January. Procedure

Question : I had a lap for endometriosis in early January. Procedure went fine and the day before I got sick I felt really good. On day 4 post op was the beginning of a mystery nightmare that no one has been able to figure out. Since then I have had 3 'episodes/flares' of violent nausea/vomiting/diarrhea/cold chills/extreme fatigue with palpitations/high heat rate at onset, the first lasted 8 days (with an additional 2 days that weren't great before discharge, then an entire 2+ weeks that I could barely function or drive at home from fatigue), the 2nd lasted 5/6 days at the worst and an additional 2 days that weren't great. and i'm on day 4 of the 3rd. With the surgery, and these episodes I have been in/out of the hospital over 20 days out of the last 3 months and no one really has any ideas that have panned out. GI, neurology, the surgeon who performed the procedure, multiple hospitalists, my rhuematologist, PCP. 2 seperate Abdomen Pelvic CT were normal, blood work mostly normal on day 1 of the issues I will have elevated WBC/neutrophils but resolves on its own. Brain CT & MRI normal. Endoscopy was negative for anything explanatory. Thyroid normal. Medication does not control the nausea/vomiting including 8mg of zofran/reglan/ativan/benadryl. Even in IV form. I have never felt this bad in my entire life.

Symptoms (1st episode):
Severe nausea (disabling)
Vomiting green bile & dry heaving
Legs twitching/hot/cold chills
Diarrhea (mostly yellow/foam)10-20x a day
I felt disoriented/weak
Ears were ringing so loud
Palpitations
Vision was off
Brain fog
Elevated WBC and neutrophils day 1 (Started to go down day 2)
And the worst symptom of all was that When the violent nausea would hit I would get so tired/out of it that I could not physically keep my eyes open.
Urine was getting darker during 2nd admission I remember the hospitalist looked at it and he thought it was so weird with me having been on fluids for so many days continuously.

Symptoms (2nd episode)
Diarrhea 2-3 days prior to onset, did have a really bad headache 1 day prior to onset
Severe nausea (disabling)
Vomiting green bile & dry heaving
Palpitations high heart rate at onset 130-150s
Legs twitching/hot/cold chills
Diarrhea during episode 5-7 times per day
Elevated WBC and neutrophils day 1
Mild ear ringing
Weak and mildly disoriented-less so than last time.
Some low center pain, moderate that would wake me up, partially relieved by bowel movement

Symptoms (3rd Episode)
Headaches worse than my normal 2-3 days before onset.
Diarrhea during episode about 5-6x/day on average
Palpitations but did not notice it was extremely high at onset like before
Severe nausea
Vomiting green bile/dry heaving
Legs less twitchy but just feel really weird/annoying.
Cold chills (cannot stay warm no matter what, shivering wakes me up)
Ears still ringing/sensitive
Weak but less disoriented feeling than before
Just feel really really bad
Normal WBC on Day 2 (no labs done day 1)
More stomach pain on/off than I had with the initial 2 episodes Mostly RUQ and low abdomen.

1)Was serum procalcitonin test done at any time(indicator of level of infection in body).Kindly go through your reports and please mention the levels at the time of intial flare up.
2)Dose of protonix you are on and from what duration.
3)RUQ pain usually have two differential diagnosis-
1)Gastritis- Which won't result in neutrophil elevation.
2)Gall bladder disease/ Infection- As a result of lapractomy.
A part from CT was a HIDA scan / MRCP or ERCP was done/Planned to check for the functions of gallbladder and Hepato-billiary symstem?
If not discuss this with your Gastro(As they are attending you physically and have correct idea about the situation).
3)Headaches,Chills,Nauseaetc can all be related to each other lose of fluid from body results in depletion of electrolytes like sodium potassium and chloride. This results in the symptoms described above but usually it can be easily corrected after using electrolytes parentrally.
Please provide in details the sequence of treatment followed after the onset of symptoms...If possible attach your Discharge notes.
I want to go through your serum electrolytes level at the time of onset of symptoms.

-This test was never done by any provider.

-I do not have my gall bladder, it's been gone since 2011. I had a MRCP done (not during flare) but it did not show anything abnormal except ansa pancreatica which we already knew I had.
3)Headaches,Chills,Nausea etc can all be related to each other lose of fluid from body results in depletion of electrolytes like sodium potassium and chloride. This results in the symptoms described above but usually it can be easily corrected after using electrolytes parentrally.
-The chills start before I vomit and continue even with 24/7 IV rehydration. This nausea is VIOLENT. The only time I have ever had nausea this severe is when I had a CSF leak after a lumbar puncture. I am 100% non functional. The vomit is almost always green. The nausea does not get better after vomiting.

Lastly Does any thing increases or decreases your symptoms? No. No medications have changed anything. It just seems to run it's course.

On a scale of 0-10 rate the pain in RUQ where 0 is minimum and 10 means intense.
-The RUQ pain is just minor/annoying maybe a 2, the lower abdominal pain is worse and wakes me up but typically dissipates after bowel movement. It's is more a 5ish.

the first attack was definitely worse and intensity is less with this compared to last though still completely incapacitating. If first attack was a 10 then second was a close 9 and this is an 8.

Why would cyclic vomiting start all of a sudden in a otherwise normal post operative patient.
Surgical trauma induced adrenal insufficiency is very very rare these days due to advanced technology.The cases were in large number in 80's and 90's but now days its rare even in developing nation...and almost a never seen thing in developed nations. Secondly your lab reports at any time were not suggestive of any sepsis/septic shock.This makes it even more unlikely.

So case summary is that post op you were ok...Then the symptoms started and for them you were given intravenous Antacids,Anti emetics etc(as for a flare up protonix 40 mg is a baseline dose and in hospitalized patient (not responding) even PPI'S are given intravenously in high doses(for pantoprazole upto 160 mg per day).
Nausea and vomiting were tried by i/v medications like Ondansetron 8mg thrice daily doses(still no change).
Initially elevated levels of blood cells were treated by patentrally administered antibiotics etc and on discharge they shifted you on tablets?

Correct me if i am wrong.
Secondly please attach your discharge papers/they contains exact treatment given to patient..See by now you must be understanding that for a genuine advice in this case detailed history of symptoms/Treatment/Duration and nature of treatment etc is very important.
I just don't want to suggest you that yes it could be adrenals go for a MRI/Suppression test...Hope you can understand it.

I was able to avoid hospital admission this time so that is the main difference. The first time was much more intense.

So case summary is that post op you were ok...Then the symptoms started and for them you were given intravenous Antacids,Anti emetics etc(as for a flare up protonix 40 mg is a baseline dose and in hospitalized patient (not responding) even PPI'S are given intravenously in high doses(for pantoprazole upto 160 mg per day).

Nausea and vomiting were tried by i/v medications like Ondansetron 8mg thrice daily doses(still no change).
Yes multiple IV medications had ZERO affect on the nausea/vomiting.

Initially elevated levels of blood cells were treated by patentrally administered antibiotics etc and on discharge they shifted you on tablets?
On day 2 of first episode I had a shot of rocephin given for suspected UTI (was proved negative on culture) continued to decline I had 3 doses of IV zithromax toward the end of it also because they thought I possibly had some type of bacterial GI bug.
On 2nd episode I had NO antibiotics and 3rd I also had none.

Correct me if i am wrong.
Just from the 1st episode I have over 500 pages from the admission (2 different hospitals) so I think its just too much honestly for you to go through.


And you have not answered this one-

Urine was getting darker during 2nd admission I remember the hospitalist looked at it and he thought it was so weird with me having been on fluids for so many days continuously. Symptoms (2nd episode) Diarrhea 2-3 days prior to onset, did have a really bad headache 1 day prior to onset Severe nausea (disabling) Vomiting green bile & dry heaving Palpitations high heart rate at onset 130-150s Legs twitching/hot/cold chills Diarrhea during episode 5-7 times per day Elevated WBC and neutrophils day 1 Mild ear ringing Weak and mildly disoriented-less so than last time. Some low center pain, moderate that would wake me up, partially relieved by bowel movement Symptoms (3rd Episode) Headaches worse than my normal 2-3 days before onset. Diarrhea during episode about 5-6x/day on average Palpitations but did not notice it was extremely high at onset like before Severe nausea Vomiting green bile/dry heaving Legs less twitchy but just feel really weird/annoying. Cold chills (cannot stay warm no matter what, shivering wakes me up) Ears still ringing/sensitive Weak but less disoriented feeling than before Just feel really really bad Normal WBC on Day 2 (no labs done day 1) More stomach pain on/off than I had with the initial 2 episodes Mostly RUQ and low abdomen.
