Question: I had sex with a HIV positive person and started PEP and while on therapy I again had sex with a high risk person for HIV. Should I extend therapy?

Before answering to your question, I would like to know few more details from your side.
1. How many hours after the first exposure, you had started your PEP?
2. What is the regimen of PEP started for you?
3. Are you strictly adhere to the regimen?
4. You had mentioned two exposures - Both are unprotected exposures?

There is no separate guidelines available in the recommended PEP, when there is another exposure is there in the midst of the course. PEP is mainly recommended for the healthcare peronnels to protect them from theor occupational risk. Nowadys it is alo recommended for sexual exposures also.
PrEP (Pre-exposure prophylaxis) is recommended for people at risk like discordant couple, Men sex with men, Female sex workers and IV drug abusers. Only certain combination of drugs will be effective for PrEP as there are lot of studies. Kindly furnish the above details to continue the discussion.

I started PEP 18hrs after the 1st unprotected exsposure. Exactly 1 Week later after strict adherence to therapy, I had unprotected sex again with a hiv+ person. But before the 2nd exsposure I took a double dose of truvada. After that I had no more exsposures and was strictly adherent to the PEP regiment. Do I need to extend therapy? I take Truvada-1X & Insentress-2X a day

Hi,
Welcome back.
As your PEP started earlier than 72 hours and ideal combination of latest recommendation of drugs had been started, you are very much safe. Moreover as you are on trivada, it would act as Pre-exposure prophylaxis also. You have to complete rhe regimen for two weeks and has to be followed up for 6 months with serological examination on 0,1,2,3 and 6 month before proclaimed to be completely safe. There is no need for the PEP to be extended. You have to use safe sex in future.
