Question : I have a sight bump on my anus and while not very painful there is some dscomfot - i have been taking 20-30 minutes on tbe toilet over the past 1 monthor so as sorry the message sent before i finished the write up. please see attached and advise. the pain is not much at this time and i have had extensive alcohol and spicy food consumption for the past 2 weeks and long bowel movements - are they related at all to this? please advise.

Hi,

It may be a hemorrhoid. A hemorrhoid is a dilated vein in the anus. The vein dilates as a result of straining to stool or long periods in the bathroom or in persons with persistent coughing or those who lift heavy objects a lot.it can be related to your diet since your diet has changed your bowel habits. Hemorrhoids can cause a swelling or mass in the anus.

Usually they can decrease in size with effective treatment.

Treatment for acute hemorrhoids- (1)dab not wipe after each bowel movement, this prevents further aggravation of the hemorrhoids
(2)using a stool softener for a few days to prevent straining eg ducolax
(3)avoid prolonged standing or sitting ( can use a cushion)
(4)use anti inflammatory pain meds to help with the discomfort eg ibuprofen or naproxen. A topical pain killer such as lidocaine can be used.

Need to reduce the foods causing the problem. High fiber foods should be used to reduce straining-fruits, vegetables, whole grain foods

thank you for your response.

what can i do to treat it - your response indicates dab only after a bowel movement - does this mean that it will go away on its own? should i use any OTC treatment or consider surgical removal - do you feel that hemorrhoids like this may be caused by my spicy food and alcohol or is it caused by more mechanical aspects - i did have a really bad timed cough with my sinus headache last week. should i be concerned about cancer? i have no other symptoms. lastly how long do they typically last from being self treated. thank you

HI,
Yes, as mentioned earlier, they do decrease with effective treatment. Dabbing after a bowel movement is to prevent further aggravation of the hemorrhoids. Wiping can irritate them which would cause further swelling Yes, if foods affect bowel movements, either increasing the frequency of bowel movements or the length of time or straining, this can cause hemorrhoids The coughing would have to be chronic to cause the hemorrhoids, this would be for at least 4 weeks.

They can normally last 1 to 2 weeks. You can also try sitz baths as well, this involves sitting in warm water (not hot) for 15 to 20 mins. The water should contain salt or epsom salt.It is an external hemorrhoids. Internal hemorrhoids are far enough insider that they cannot be seen or felt

thank you - what can i do to achieve %E2%80%98effective treatment%E2%80%99 - should i get it surgically removed? can i travel and continue running and yoga? sorry for so many questions but i want to take the necessary action i need to in order to resolve this - thank you

No, surgeons refer the conventional treatment mentioned above, surgery is usually a last resort.

As mentioned- (1)sitz baths to sooth and decrease inflammation and swelling
(2)dab with bowel movements to reduce inflammation and swelling
(3)avoid prolonged sitting or standing, use a cushion when sitting
(4)oral anti inflammatory pain meds such as naproxen or ibuprofen
(5)stool softener for a few days to reduce straining
(6)avoid foods that can cause a problem

Use a high fiber diet with fruits/vegetables and whole grain foods plus enough daily liquid (8 glasses) to avoid recurrence

Exercise is fine. Travelling does not affect hemorrhoids.

Hi,

I understand you are very busy but i have other questions in my querries and it is frustrating to have to repeat them given that this is a paid service. i have major responsibilities abd trying to take action to resolve this condition. i have questions around exercise, how long this will take typically and other questions above etc..

HI

The treatments mentioned are what would normally be told if a patient visited their doctor's office. If you were to look at medical treatment of hemorrhoids on a doctor website like medscape that writes information for doctors on how to treat patients, it would tell you the same treatments.

This is the treatment that I and any GP or general surgeon would initially recommend to treat hemorrhoids, surgery is not first line. Surgery is only used as a last resort.

Treatment of hemorrhoids involves-(1)using a lot of fluids and high fiber diet- this prevents straining. Straining causes or worsens the hemorrhoids
(2)avoiding prolonged standing or sitting- sitting for too long hurts hemorrhoids and standing means a lot of pressure for a long period
(3)sitz baths- this works so well for hemorrhoids, the warm water soothes them and the salt helps to shrink them
(4)oral medication- the medication mentioned- ibuprofen and naproxen, as mentioned are anti inflammatory, they therefore reduce swelling and pain
(5)stool softener for a few days so that there is no straining which would worsen them
(6)avoiding foods that worsen the problem


If they do not respond to the above or they are very painful only then is surgery suggested, it is not firstline for the majority but you can see your GP and see if he or she thinks that you are a candidate for surgery if you prefer that option.

I have a number of patients with hemorrhoids and the reason that i keep repeating the same treatment is because they work well for all the patients i have told this treatment plan too.
