Question: I have a very complicated medical history. i have this throbbing lower back pain on and off forvalmost a year. Now getting more frequent and more intense.

Hello, I'm Dr. Branch, thanks for using 'Ask a Doctor'. Typically lower back pain is caused by musculoskeletal causes, in which case conservative management is recommended initially, with pain control (such as with Tylenol, Ibuprofen, and possibly other pain medications such as cyclobenzaprine) and physical therapy. If you are still having pain, x-rays may be done, as well as blood tests to check for inflammation that might be going on and be related to the back pain.

During the initial evaluation, if there was any concern for cancer or infection, imaging may be considered at that point, either with x-rays, or possibly with an MRI of the spine. Also, if any neurological damage was seen on examination, an MRI would likely be considered.

Rarely, a patient may have damage to the spine that would justify considering other interventions, such as spine injections or even surgery. Usually this would only be considered after conservative management has been tried for several months at least.
